Comments:
We ask you to consider the possibility of providing these services and, in case of a positive decision, send a commercial offer consisting of:
1. A cover letter indicating the cost and detailed conditions for the provision of services;
2. Completed offer card;
3. A completed questionnaire of the beneficiary from the head and all the founders of the organization;
4. In accordance with the security policy of Polymetal Holding, as part of the submission of a commercial offer, the Bidder must also provide the following documents:
 - Consent to the processing of personal data issued on letterhead and sealed in the name of the Managing Director of Kutynskaya GGK LLC Tarabarova Vasilina Valentinovna from the head and all the founders of your organization;
 - The Charter (in the latest edition) with all amendments and additions;
 - Certificate of state registration (on assignment of OGRN);
 - Certificate of tax registration;
 - Decision of the authorized management body of a legal entity on the appointment of an executive body;
 - Letter from the Federal State Statistics Service on the assignment of codes;
 - Extract from the Unified State Register of Legal Entities;
 - Accounting (financial) statements for the last reporting year, or for the last reporting period;
 - Bank statement on the movement of funds for the last 3 months;

This request for proposal procedure is not a tender and its conduct is not regulated by Articles 447-449 Part 1 of the Civil Code of the Russian Federation;
This procedure is also not a public competition and is not regulated by Articles 1057-1061 Part 2 of the Civil Code of the Russian Federation;
Thus, this request for proposal procedure does not impose on the Customer an appropriate amount of civil obligations.
Venue of bidding:This call for bids is held in electronic form on the e-marketplace of B2B-Center group (www.b2b-center.ru). Bids shall be submitted in the form of e-document.
