The composition of the application for participation: 1. An extract from the Unified State Register of Legal Entities/USRIP (received no earlier than 6 months before the date of submission of the application for participation in the procurement procedure); 2. A completed commercial offer (NECESSARILY WITH PAYMENT TERMS AND DELIVERY DATES); 3. Dealer credentials, certificates; 4. A reference list for completed deliveries of similar products (with contact details); 5. Brief information about the company; 6. Accounting statements (F01 balance sheet, F02 profit and loss statement); 7. Delivery guarantees; 8. Information about the owners of the legal entity, a list of affiliated persons and organizations of the supplier and the head; 9 .Agreement with the draft agreement.
