7. Requirement to the contractor:
7.1 Carrying out work on lining the lid of the vault of the re-firing furnace No. 3 TSPOIP inv. No. 6469 is made from the Customer's material. (Fireclay heat-insulating refractory products SL-23 STO 05802307-0-002-2014 and heat-resistant mastic MT-1)
Equipment according to the PPR, tools and equipment necessary for carrying out the entire complex of works are provided by the Contractor.
7.2 Working in the conditions of an operating enterprise. The operating mode of the enterprise is round the clock
7.3 Harmful factors may be affected: dustiness, gas contamination
7.4. Development of PPR and technological maps for all necessary work.
7.5. The Contractor must have the necessary production and human qualified
resources to perform this work. Have the certification of the head, ITR and workers on

7.12. Development and approval of linear schedules for furnace repairs.
8. Payment terms: monthly, within 30 (thirty) banking days after the delivery
of the results of the work, provided that the work is done properly and within the agreed time. The works are considered completed after the Parties sign the Act of acceptance of the completed works (form COP-2) and a certificate of the cost of the completed works and costs (form COP-3).
9. Terms of work: The approximate date of the work is July 2022. The beginning and end of the work are agreed with the Customer.
10. Warranty obligations for the work performed: 24 months from the date of completion of the work, for each of the repaired objects.
11. The transfer for repair and acceptance of equipment from repair is carried out in accordance with the REGULATIONS approved at EPM-NEZ JSC.
12. Repair of furnaces is carried out in the operating workshop. The repair schedule is coordinated with the workshop (TSPOIP) and the plant services (UM, EU).

2. copies of documents confirming the presence of a service for

contracts with an accredited organization in the field of labor protection services;
3. a copy of the instruction on labor protection from the instruction log;
4. POR or PPR or RTI;
5. a copy of the protocol of certification of workers on labor protection;
6. copies of certificates of certification of ITR for labor protection and general requirements

under pressure, using GPM, in electrical installations, fire works

The Contractor notifies the Customer about the fulfillment of all the specified conditions in writing
the form with the attachment of all supporting documents. Non-fulfillment of conditions
this paragraph by the Contractor gives the right to the Customer not to allow the Contractor to

14. Note:
- Delivery of the material from the warehouses of JSC "EPM-NEZ" to the place of work, garbage removal (non-slagged refractory bricks, graphite, scrap metal, etc.) to the storage sites located on the territory of JSC "EPM-NEZ" is carried out by the Contractor's transport.
- Garbage removal of all categories to landfills for receiving industrial waste located outside the territory of JSC "EPM-NEZ" (garbage removal at a distance of 7km. to 30km.) is carried out by transport provided by the Customer, upon prior request of the Contractor.
- Waste disposal costs of all categories, at landfills receiving industrial
waste located outside the territory of JSC "EPM-NEZ" is carried out at the expense of the Customer.
- Overhead cranes for work on furnaces are provided by the Customer, the driver
the Contractor provides a bridge crane to perform the work.

- a copy of the protocol of certification of workers on labor protection;
- copies of certificates of certification of ITR on labor protection and on general requirements of industrial safety. When performing work on gas, on pressure vessels, using GPM, in electrical installations, fire works – copies of certificates for each type of work;
- medical admission of personnel - copies of certificates of gas and electric welders, slingers, crane operators, lifts (towers). For gas and electric welders, copies of the PTM certification protocols.
In case of detection of inconsistencies of the information in the submitted documents with the requirements of regulatory legal acts, the Customer has the right to initiate an audit by the Health and Safety Service in matters of Occupational Safety and Industrial Safety in the Contractor's organization.
The Contractor notifies the Customer of the fulfillment of all the specified conditions in writing with the attachment of all supporting documents. Failure to comply with the terms of this clause by the Contractor entitles the Customer to prevent the Contractor from performing the work, the delay in performing the work in this case is the fault of the Contractor
