2. The purpose of the work.
2.1. Ensuring the effective operation of ventilation from the graphite furnaces of section No. 2 of the CGE.
3. Scope of work performed:
3.1. Dismantling, manufacturing, installation according to the Requirements Specification and the statement of scope of work No. 16-07/23 (Appendix No. 1 of the Requirements Specification)
3.2. Cleaning of the place of work.
4. Requirements for the Contractor.
4.1. The contractor must have all the necessary production and human resources to perform this work.
4.2. In order to clarify the conditions of work, familiarization with the object, the arrival of the Contractor's representatives to the Customer is necessary.
4.3. The Contractor in his commercial offer indicates the payment procedure for the work and other conditions.
4.4. Certification of the manager, IT and executors of industrial safety and labor protection at the OPO according to Appendix No.2.
4.5. Provision of measures for occupational health and safety of safe methods of work at the facility.
4.6. In case of justified comments to the staff on the quality of the work performed, violation of safety regulations, as well as violations of an administrative nature, at the request of the Customer's specialists, the Contractor is obliged to immediately replace these employees.
4.7. The work not performed qualitatively must be redone at the expense of the Contractor and handed over to the Customer.
4.8. Placement in sanitary premises of workers employed to perform this work is carried out by the Contractor. The customer, if possible, can arrange the placement of workers on a contractual basis.
4.9. The Contractor must develop and coordinate with the Customer estimates, a linear schedule of work, POR, PPR and technological maps for all necessary work – within no more than 15 calendar days after the announcement of the winner of the competitive procedure.
4.10. Provide certificates of conformity for the materials used.
4.11. It is necessary to have SRO for these types of work.
5. Working conditions
5.1. Work at an existing enterprise.
5.2. Works are performed from the Contractor's material. Equipment - smoke pump DN-21 is provided by the Customer.
5.3. Lifting mechanisms, mechanisms for loading and unloading operations, vehicles, special equipment for work and delivery of finished products to the workshop are provided by the Contractor.
5.4. Electric welding and gas-cutting equipment and consumables for them are provided by the Contractor.
5.5 Vehicles for the removal of garbage, scrap metal or other waste are provided by the Contractor.
6. Place of work:
6.1. Territory of the Customer, CGE section No. 2.
7. Terms of work:
7.1. Works are carried out according to the agreed schedule.
8. Terms of payment:
8.1. Payment is made after the actual execution within the agreed time frame.
9. Warranty period.
The Contractor provides the Customer with a guarantee - 12 months from the date of commissioning.
10. Mandatory requirements and conditions for the conclusion of the contract.
10.1 If the number of the contractor's personnel (including the full-time number and the number of employees engaged in labor activity on the basis of civil law contracts) is less than 10 (ten) people, the contractor's head is obliged, prior to the commencement of work under the contract, to submit in two copies an appeal containing the name list of the personnel whose forces it is supposed to perform works under the contract. Copies of civil passports and employment contracts (or civil law contracts) concluded with the persons indicated in the application must be attached to the said application.
10.2. Based on the results of the work performed under the contract, the contractor, together with the KS-2 act, provides the customer with copies of the report of the SSV-M Form "Information on Insured Persons" and a positive protocol of input control on the transfer of the said report to the territorial body of the Pension Fund of Russia, signed by the E-Signature, for the period of time specified in the act submitted by him forms of KS-2.
10.3. In case of non-submission of the reports of the SSV-M, or a discrepancy in the smaller side of the number of employees of the contractor specified in them, with the number indicated in the previously submitted appeal, the acts of the CS-2 by the customer are not signed, and the parties to the contract acknowledge the absence of the fact of the contractor's work, the work on the specified acts is not subject to payment.
11. Requirements for the submitted proposals.
11.1. The Contractor shall specify in the Technical and Commercial Proposal the terms of the work, the cost of the work, as well as the terms of payment.
11.2. Prerequisite: the Technical and Commercial Proposal must be accompanied by:
- estimated calculation of the amount of work specified in the project documentation;
- schedule of work.
12. The application.
Appendix No. 2
The list of documents provided to the OTiPB for registration of work permits for the production of works by contractors:
1. A copy of the Certificate (SRO) for the right to perform this type of work.
2. A copy of the instruction on labor protection from the journal of instruction, POR, PPR or Fabricated rubber products.
3. A copy of the protocol of certification of workers on labor protection.
4. Copies of certificates of certification of ITP on labor protection and on general requirements of industrial safety. When performing work on gas, on pressure vessels using GPM, lifts (towers), in electrical installations, carrying out fire work - copies of certificates for each type of work.
5. Medical admission of personnel.
6. Copies of certificates of gas-electric welders, slingers, operators of cranes, lifts (towers). For gas- electric welders - copies of the fire-technical minimum certification protocols.
7. Prior to registration of the work permit, the Contractor is obliged to provide the Customer with the following documents: copies of documents confirming the presence of a labor protection service and/or a labor protection specialist in the contractor (an order of appointment to a position in the contractor, a certificate of a labor protection specialist) or the existence of an agreement with an accredited organization in the field of rendering labor protection services.
8. Before the start of registration of the work permit, all employees of the contractor, including managers, are required to undergo an introductory briefing at the OTiPB (to have a document confirming their identity and age with them) When performing work, all employees must have certified clothing, shoes and other personal protective equipment necessary to perform this type of work.
Note: In case of expiration of any document, the work permit is not issued until a new document is submitted.
