2. All forms of documents are obligatory for filling. To participate in the procurement procedure, you must complete and provide:
2.1 Map of the proposal and its annexes;
2.2 Applications № № 1, 2 to the contract template for the execution of works:
- Annex 1 calculation of the contract price,
- Annex 2 Schedule of work execution;
2.3 Extract from the Register of SRO members on the AGC with tolerances in accordance with the requirements of TK;
2.4 Documentation on workers (industrial safety (attestation in RTN), labor protection, fire-technical minimum, work with the GPM, certificates Naks (for the ITR minimum 2 level, for the head of Welding production 3 level, for welders 1 level );
2.5 Certificates and certificate of welding equipment in accordance with the requirements of the legislation of the Russian Federation (order RTN № 5 from 13.01.2015) at the facilities of the OGP;
2.6 Letters of recommendation (in case of availability);
2.7 Package of constituent documents:
- Certificate of State registration (on the assignment of UAH);
- Certificate of tax registration;
- Letter of the Federal Service of State statistics on attribution of codes;
- Decision of the authorized body of the legal entity on the appointment of the Executive Body;
- Power of attorney for the person to whom the contract will be signed (if necessary);
- Charter of a legal entity (in the last edition with all amendments and additions);
- Extract from the unified State Register of legal entities issued by the Russian Federal Tax Administration (statute of limitations not exceeding 1 calendar month);
- Accounting (financial) reporting for the last reporting year, or for the last reporting period, if the counterparty makes interim accounting (financial) reporting, with the application of evidence of reporting to the Tax Inspectorate;
- Bank statement on the flow of funds for the last 3 months;
- Consent to the processing of personal data
- Copy of Passport, insurance Certificate of State pension insurance-for natural persons.
3. Proposals will not be accepted without loaded documentation (completed forms of documents, annexes according to paragraph 2).
4. Criteria for selecting the best offer: Availability of necessary documents for performance of works, terms and cost of performance of services, terms of payment, presence of work experience.
5. This procedure of request for quotation is not a contest and its conduct is not regulated by articles 447-449 of the Civil Code of the Russian Federation;
6. This procedure is also not a public contest and is not regulated by articles 1057-1061 of the Civil Code of the Russian Federation;
7. Thus, this procedure of request for proposal does not impose on the customer the corresponding volume of civil obligations.
8. The attached contract is not a public offer, but the terms of the contract will be binding on the winner of the bidding.
