In addition, monitoring participants are encouraged to complete and attach the price table (available on the Monitoring Documentation tab).
The Service Price Calculation Table form is filled out by the monitoring participant and uploaded to the ETP if it is possible to provide such a calculation (the form is posted as part of the documentation monitoring).
