Comments: The best offers will be determined during the procurement procedure. As part of the pre-qualification selection, each participant must attach to his application on the electronic platform www.b2b-center.ru , and additionally send to e-mail e.zozulya@x5.ru the following documents:
• Appendices No. 1, 2, 3, 4 – filled in, in the original format;
• Scanned copy of the statement of the Unified State Register of Legal Entities (EGRIP) issued by the inspectorate of the Federal Tax Service of the Russian Federation (no later than a month ago at the time of submission;
• Scanned copy of the decision on the appointment of the head of the Company (minutes of the meeting of participants (shareholders) of the Company, the decision of the sole founder);
• Scanned copy of the power of attorney for the person entitled to conclude and sign the Contract on behalf of the Company (if any);
• Scan-a copy of the existing license to carry out medical activities, in case of involvement of third parties in the performance of services under the contract – the relevant licenses and documents confirming subcontracting relationships (contracts);
• Scanned copy of the certificate of ownership or lease agreement of the building(s)/premises(s) of the medical center and the document(s) confirming their compliance with sanitary and epidemiological standards;
• Scanned copy of documents confirming the ownership or long-term lease of a mobile digital fluorography cabinet and a mobile medical complex; registration certificate of a medical device and a certificate of a vehicle (for the price segment of St. Petersburg and LO);
• A letter of guarantee on the availability of a sufficient number of qualified medical specialists in the state.
Attention! Participants who have not attached a complete set of documents on the electronic platform or documents filled out incorrectly are not allowed to the second stage of the procedure and do not participate in the further selection.
The Customer provides the Participating Companies with a standard contract for the provision of medical services, which in the proposed version is mandatory for cooperation (the contract is contained in Annex No. 5 to this Invitation), for familiarization and decision-making on the readiness to sign.
Venue of bidding:
This call for bids is held in electronic form on the e-marketplace of B2B-Center group (www.b2b-center.ru). Bids shall be submitted in the form of e-document.
