Delivery terms:All services are provided at the same time throughout the lot from 01 to 03 August 2023. on the territory of LLC Volgogradservice. The exact number of employees is indicated when signing the contract for the provision of services, the number of employees subject to medical examination is approximately 200 people.

Terms of reference for the service provider for
periodic medical examination of employees of Volgogradservice LLC in 2023.
Requirements for the service provider.
1. Activity in the market of providing medical services for at least 3 years.
2. The Contractor has a license to carry out medical activities, laboratory services.
3. Conducting a medical examination on the territory of Volgogradservice LLC at the address: Volgograd, ul. 40 Let VLKSM 55.
4. Conducting a medical examination on the territory of Volgogradservice LLC with the use of mobile diagnostic complexes.
5. The presence of all specialists in the commission for conducting medical examinations on the Customer's territory for conducting surveys on harmful and (or) dangerous production factors and (or) types of work, according to the Appendix.
6. Conducting a medical examination on the territory of Volgogradservice LLC in the period from 01 to 03 August 2023.
7. Processing of the results of periodic inspection no more than 20 calendar days.
8. Providing the Customer with the following documents based on the results of a medical examination: a conclusion for each employee, a health passport for each employee, a Final Act common to all employees who have passed a periodic medical examination.
9. The number of employees subject to medical examination is approximately 200 people.
10. The possibility of additional examination of the Customer's employees, after a medical examination, in the Krasnoarmeysky district of Volgograd.
11. Approval of the Final Act based on the results of a periodic medical examination in the territorial administration of Rospotrebnadzor in the Volgograd region – by the Contractor.
12. Deferred payment based on the results of the inspection, no more than 63 days.

2. To their proposals in electronic form, the participants of this procedure MUST, before opening the envelopes, attach the applicant's Questionnaire and documents, according to the list, simultaneously with the submission of the proposal and a file in the form of a scanned copy of their commercial proposal signed by an authorized person and stamped by the organization participating in this procedure. In case of non-fulfillment or improper fulfillment of this requirement by the participant of the procedure, the organizer of the procedure reserves the right not to consider the proposal of this participant.
3. The cost of services should be unchanged for the whole of 2023. (written confirmation).

Thus, this procedure does not impose an appropriate amount of civil obligations on the Organizer and reserves the right to refuse all Proposals received for any reason or terminate the selection procedure at any time, without incurring any responsibility to the Participants.
