7. Documents confirming the authority of the head of the legal entity and his representatives:
a. A certified copy of the decision authorized by the management body of the organization on the appointment of a particular person to the position of the head of the organization (or the protocol of the higher collegial body, or order of the sole superior authority),
b. The information on the surname, first name, patronymic and name of the position of the head specified in this decision must correspond to the extract from the Unified State Register of Legal Entities.
