Comments:
Request for proposals will be held from 26.11.2019g. on 28.11.2019g. 14:00 (Moscow time), without the possibility of prolongation after the normal time in accordance with the rules of Bid (Appendix №2), without initial rates, without displaying the best rates. A requirement for participation in the request for proposals is attached on the electronic platform of the final proposal on the basis of Request for Proposals (certified signature of the responsible officer and the seal of the organization), indicating: price for each lot in rubles without VAT 20%.

• qualification (Appendix №5 tender documentation) into Excel and PDF format (certified signature of the responsible officer and stamped) by downloading the documents on ETP;
• letter - agreement with the terms of reference conditions and willingness to sign a standard form of contract (in free form. On the organization's letterhead) Load the document on the ETF;
• license to carry out preliminary and periodic medical examinations
