Comments: Your technical and commercial proposal must be signed by a person who, in accordance with the law, has the right to act on behalf of the contractor without a power of attorney / or a person duly authorized by him on the basis of a power of attorney (hereinafter referred to as the authorized person) and must contain:
1. Participant's questionnaire.
2. Application for participation in the tender indicating the terms of payment for the work performed, the warranty period, the total cost of repair work.
3. Appendices No. 3,4 on health and safety.
4. Estimated calculations.
5. Detailed commercial offer with all accompanying documentation.
LLC "ISO" in Achinsk reserves the right:
1. reject an offer if:
1.1. the selection participant does not have professional knowledge and does not have all the types of resources necessary for the fulfillment of the contract (production capacity, equipment, labor resources);
1.2. the selection participant does not have the necessary licenses and certificates of admission to this type of work;
1.3. revealed the facts of provision by the Selection Participant of inaccurate information;
1.4. the selection participant is in the process of liquidation or reorganization, or insolvency (bankruptcy) proceedings have been initiated against him;
2. not to motivate your decision to determine the Selection Winner;
In case of victory in this selection, before concluding an agreement, you must provide copies of the following documents:
- Notarized copies of powers of attorney of authorized persons who have the right to sign Acts of Work Completed with a sample signature;
- Certificate of state registration in the Unified State Register of Legal Entities;
- Certificate of registration with the IFTS;
- Notices of registration with the IFTS as the largest taxpayer (if the Counterparty is registered as the largest taxpayer before 2006, then the Counterparty provides a Certificate of registration as the largest taxpayer);
- Extracts from the Unified State Register of Legal Entities as of the date not earlier than 10 working days before the date of sending the Agreement for approval;
- Documents confirming the powers of the persons signing the Agreement and primary documents (orders for the appointment of a director and chief accountant; powers of attorney containing the appropriate powers);
- Accounting statements (Balance sheet (Form No. 1) and Profit and Loss Statement (Form 2)) for the last reporting date preceding the date of conclusion of the Agreement, with a mark on the acceptance of the Federal Tax Service Inspectorate (including with the attachment of a receipt for acceptance by TCS, if the company reports to the IFTS in electronic form);
- Certificates from the Federal Tax Service Inspectorate (Form 39-1) as of the date, not earlier than 10 working days before the date of sending the Agreement for approval;
- Constituent documents;
- Licenses required for the performance of the Agreement.
In case of victory in this selection, you must, prior to the start of the work, ensure that the employees of the organization undergo a preliminary medical examination at RUSAL Medical Center LLC (branch in Achinsk). Workers are allowed to perform work who do not have medical contraindications for the work performed, as well as who have qualification certificates for the main and related professions. Employees who have undergone a preliminary medical examination in medical and preventive institutions of the city undergo a medical examination at the Branch of RUSAL Medical Center LLC in Achinsk to form a risk group. The Contractor's employees included in the "risk group" based on the results of medical examinations may be allowed to work in the future only after passing the pre-shift medical examination-admission to LLC RUSAL Medical Center (branch in Achinsk).
The selection participant can visit the object, inspect it and collect the necessary information that is required to prepare the Bid. The customer will provide the necessary assistance to the visitors. All expenses related to the visit to the object are borne by the Participant independently.
The presence of the Participant on the site for any purpose, with or without the approval of the Customer, will be carried out at full risk for the Participant, including death, injury, loss or damage to any property, as well as any costs incurred in this regard.
Any information collected by the Participant at the site is confidential and cannot be disseminated by the Participant to third parties without the consent of the Selection Organizer (Appendix No. 8 to this letter)
