3. Place of performance of works / services: Health center of JSC "EPM-NEZ", located in a part of the non-residential premises, located at the address: Novocherkassk, Aluminum site, in the building Liter Zh-V, on the 1st floor, with a total area of 115.4 sq.m. Premises, medical equipment, instruments are transferred under a separate contract for the lease of non-residential premises. The contractor has the right to use his own equipment necessary for the high-quality provision of medical services.
3.1. The operating mode of the health center is round-the-clock (24 hours / 7 days a week), the provision of services on weekends and holidays.
3.2. Personnel, requirements for the Contractor's personnel
The Contractor provides medical services to its employed personnel in accordance with the Labor Code of the Russian Federation. Recommended staff:
1. Head of the medical center, therapist-1.0 rate, Working hours Mon-Fri from 8-00 to 17-00
2. Paramedic - 1.0 rate, Working hours Mon-Fri from 8-00 to 17-00
3. Paramedic - 4.0 rates, Schedule of work Mon-Mon from 8-00 to 8-00
4. Nurse - 1.0 rate, Work schedule from 8-00 to 17-00
5. Head nurse - 1.0 rate, Schedule from 08-00 to 17-00
6. Physician of a narrow specialty upon request - 1.0 rate, work schedule as agreed with the Customer.
Doctors or paramedics who are employees of a medical institution that has an appropriate license and contractual relationship with EPM-NEZ JSC should be involved in work in health centers.
Only employees who have personal medical records and who have timely passed periodic professional training and certification, have passed periodic and / or preliminary medical examination when applying for work are allowed to provide services.
The Contractor is obliged to send, at the request of the Customer, the medical personnel involved in the provision of services for educational trainings; certify in accordance with applicable law.
The contractor is obliged to provide replacement personnel for the period of absence of the main body of medical workers. Replacement personnel must meet all the requirements of section 3 of this technical assignment.
Delivery of personnel to remote production facilities is carried out by the Customer's transport.
7. CHARACTERISTIC AND SCOPE OF PERFORMED WORKS / SERVICES
7.1. The scope of services provided:
The service provider needs to organize on the territory of the Customer the uninterrupted operation of the health center (according to the operating mode), in which the Contractor's forces and means will provide primary medical pre-medical medical and sanitary care to employees at the production facilities of the Customer, in accordance with the requirements of the Legislation of the Russian Federation, as well as requirements of local regulatory documents of the Customer, provision of therapeutic assistance and carrying out pre-shift and post-shift medical examinations of employees of EPM-NEZ JSC and contractors.
7.2. Types of medical care and medical services provided:
- Primary health care at the prehospital stage for sick workers or victims of accidents;
- Routine medical care - primary pre-medical and / or medical health care;
- Examination by a medical professional using a breathalyzer valid at the time of testing for intoxication (alcoholic, narcotic or other condition associated with the use of psychoactive intoxicating substances by employees);
- Organization and implementation of preventive measures at the facilities of the Company and the medical institution of the Contractor (according to Table 1) aimed at reducing the incidence of diseases and promoting a healthy lifestyle
- Training of employees in the provision of emergency first aid medical care in case of industrial accidents;
- Conducting sanitary and educational work among employees;
- Prophylactic vaccination of employees to prevent the emergence and spread of infectious diseases;
- Organization of medical advice for narrow specialist doctors
- Organization, if necessary, of on-the-job treatment in a health center, including physiotherapy treatment.
- Provision of outpatient and polyclinic services to employees: outpatient and polyclinic reception of a therapist, doctors of narrow specialties - on request, functional diagnostics (ECG), ultrasound diagnostics. The provision of laboratory diagnostics services and the issuance of certificates of incapacity for work is encouraged.
At the request of the Customer, the Contractor must provide documents confirming the costs of the scope of services specified in the request.
5. REQUIREMENTS FOR ORGANIZATION OF WORKS / SERVICES ON THE TERRITORY OF THE CUSTOMER'S PRODUCTION FACILITY
5.1. Requirements for the Contractor when equipping health centers:
- Medical equipment must be ready for use at all times. It is necessary to carry out regular checks of the condition of medical equipment in accordance with the requirements of the operating instructions (letter of the Ministry of Health of the Russian Federation dated October 27, 2003 No. 293-22 / 233 "On the introduction of the Methodological Recommendations" Maintenance of medical equipment "). The results of equipment checks should be documented. Supply of drugs and consumables and storage control in accordance with the requirements of the order of the Ministry of Health and Social Development of the Russian Federation dated 23.08.2010 No. 706n "On Approval of the Rules for the Storage of Medicines".
- Medical workers of health centers must be provided by the Contractor with the necessary medical clothing, footwear and personal protective equipment in accordance with the sanitary and hygienic and production requirements presented when leaving for production facilities;
- In the health center, an ambulance box must be formed, completed in accordance with the order of the Ministry of Health of Russia dated 07.08.2013 No. 549n "On approval of the requirements for the packaging of drugs and medical devices for emergency medical aid boxes and kits." The box for the provision of emergency medical care must be provided with a list of contents indicating the quantity and expiration date of medicines, medical equipment and consumables;
- Health care providers should regularly check the expiration dates of medicinal products and maintain an appropriate database;
- Provision of the Contractor's personnel with office supplies, reporting logs, in accordance with the local regulatory documents of the Customer, transferred under the Acceptance and Transfer Certificate of local regulatory documents;
- Provision of personnel with cleaning and detergents, personal protective equipment;
- Delivery of medicines, vaccines and supplies to the health center by the Contractor;
- Removal and disposal of medical waste by the Contractor.
5.2. Requirements for the Contractor.
The contractor must have:
- office and medical facility in the Rostov region;
- a license to carry out medical activities for the requested services (according to the terms of reference);
- the required number of qualified medical personnel (in accordance with the requirements of the technical assignment);
- contractual relations with suppliers of certified medicines and consumables, with enterprises providing services for the repair, verification and maintenance of medical equipment;
- contractual relations with enterprises providing services for the removal and disposal of medical waste;
- a valid contract for production control services in accordance with SanPiN 2.1.3.2630-10;
6. QUALIFICATION REQUIREMENTS, SKILLS AND RESPONSIBILITIES OF THE MEDICAL PERSONNEL
6.1. Requirements for the qualifications of medical workers:
A paramedic must have a secondary or higher medical education, valid specialist certificates of the established sample, work experience of at least 5 years.
Head of the health center, physician-therapist, higher medical professional education in the specialty "General Medicine", valid certificates of a specialist of the established standard in the specialty "Therapy", work experience of at least 5 years. Specialization in the specialty "Occupational Pathology" is encouraged.
A doctor of narrow specialization - higher medical professional education in the specialty "General Medicine", valid certificates of a specialist of the established standard in the specialty "Therapy", education in the field of activity, work experience of at least 5 years.
Senior nurse - secondary or higher medical education in the specialty "General Medicine", "Nursing", valid certificates in physiotherapy, functional diagnostics.
6.2. Skills and responsibilities of medical personnel:
A paramedic, head nurse, head of a health center, a general practitioner must:
- Possess modern knowledge and practical skills in the provision of emergency medical care in accordance with the order of the Ministry of Health of Russia dated November 13, 2012 No. 911n "On approval of the procedure for the provision of medical care in acute and chronic occupational diseases", Order of the Ministry of Health and Social Development of Russia dated 05.15.2012 No. 543n "On approval of the Regulation on the organization of the provision of primary health care to the adult population ", by order of the Ministry of Health and Social Development of Russia dated 03.23.2012 No. 252n" On approval of the Procedure for imposing on a paramedic, a midwife by the head of a medical organization in organizing the provision of primary health care and emergency medical care of certain functions of the attending physician in direct provision of medical care to the patient during the period of observation and treatment, including the appointment and use of drugs, including narcotic drugs and psychotropic drugs ", by order of the Ministry of Education and Science of Russia dated 05/12/2014 No. 514 "On the approval of the federal state educational standard of secondary vocational education in the specialty 31.02.01 General medicine";
- Have knowledge of the control of the sanitary and hygienic and anti-epidemic state and the provision of catering points and utility rooms in shift housing and production facilities in accordance with SanPiN 2.3.6.1079;
- Have knowledge of the requirements for the maintenance of equipment and storage of medicines in accordance with the requirements of the order of the Ministry of Health and Social Development of the Russian Federation dated 23.08.2010 No. 706n "On approval of the Rules for the storage of medicines");
- Have the skills to train workers in first aid;
- Provide emergency medical care for trauma and acute illness;
- Know and be able to apply the standards of cardiopulmonary resuscitation in case of sudden cardiac arrest, acute respiratory failure, allergic, coma, with various types of asphyxia, drowning, electrical injury;
- Be able to ensure the patency of the upper respiratory tract using alternative methods;
- Know and be able to apply protocols for diagnostics and emergency care for cardiovascular diseases (including automatic defibrillation), respiratory tract diseases, diseases of the abdominal cavity, endocrine diseases, blood diseases, allergic diseases, mental illness, infectious diseases;
- Know the basics of diagnostics and emergency care for injuries, injuries and poisoning, including the organization and conduct of medical triage in case of massive injuries;
- Immediately inform the responsible employees of the Company about all cases with the provision of medical assistance by the personnel of health centers in case of accidents at work;
- To be able to take and read the ECG for the underlying pathology;
- Be able to determine the blood glucose level with portable glucometers;
- Be able to conduct pulse oximetry with data interpretation;
- To be able to conduct the level of total blood cholesterol with portable express analyzers;
- Know and be able to apply methods of prescribing drugs, including potent (where applicable), indications and contraindications for their appointment, doses of drugs, possible side effects and methods of their correction;
- Know the safety precautions when working with equipment and medical gases;
- Know the drug treatment for circulatory arrest;
- Be able to sanitize the tracheobronchial tree;
- To be able to conduct oxygen therapy (indications and contraindications, conditions of conduct);
- To be able to carry out local anesthesia, primary wound care, stopping external bleeding, anterior tamponade for nosebleeds;
- To be able to carry out tube gastric lavage, bladder catheterization;
- To be able to apply an occlusive dressing in case of open pneumothorax, to aspirate air from the pleural cavity with a tension pneumothorax in accordance with the order of the Ministry of Health of Russia dated 12.24.2012. No. 1407n "On approval of the ambulance standard for spontaneous tension pneumothorax";
- Have the skills of immobilization in case of fractures of bones, spine, prolonged compression syndrome;
- Know and be able to practice the Medical Emergency Plan;
- Maintain medical records, including confidential documents, in accordance with the requirements of Federal Law No. 323-FZ of 21.11.2011 "On the Basics of Health Protection of Citizens in the Russian Federation";
- Maintain statistical reports on the work of the health center on a monthly basis, consolidate quarterly, annually by the 15th day of the month following the reporting month, in the form of a consolidated report and transfer to the head of the structural unit;
- Maintain all the necessary medical documentation provided for by the sanitary and epidemiological requirements for organizations carrying out medical activities;
- Maintain medical records and submission of reports in accordance with the established forms in accordance with the normative documentation of the Customer (section 6 of the technical task);
- Take part in the implementation of measures for the prevention of infectious and non-infectious diseases as agreed with the Customer;
- To take part in the conduct of the command-staff complex exercises conducted by the Customer;
- Participate in the development of the scenario and testing of the emergency medical response plan at the Customer's production facility (participation in conducting medical training alerts);
- Conduct pre-shift / post-shift, pre-trip / post-trip examinations;
- To be able to conduct a medical examination to identify the state of intoxication in accordance with the order of the Ministry of Health of Russia dated July 14, 2003 No. 308 "On medical examination for the state of intoxication";
- Suspend employees of the Company and contractors from work due to health conditions;
- Be able to carry out seasonal vaccination against influenza and tick-borne encephalitis;
- Maintain the premises of the health center, equipment and other property transferred by the Company to the Contractor in proper condition in accordance with the requirements of sanitary and epidemiological rules and regulations. All premises, equipment, medical and other equipment must be kept clean. Wet cleaning of premises (treatment of floors, furniture, equipment, window sills, doors) should be carried out at least 2 times a day using detergents and disinfectants approved for use in accordance with the established procedure;
- Ensure correct disposal of medical waste in accordance with SanPiN 2.1.7.2790;
- Have the skills of working on a personal computer at the user level;
- Once every 5 years, confirm your qualifications, undergo trainings on modern standards for the provision of emergency medical care (an extended complex of resuscitation measures) on the basis of a medical institution or a medical educational institution (center) with the receipt of a certificate / certificate of an arbitrary (non-state) sample on training at the training on basic and extended life support, extended resuscitation measures for cardiac arrest, with trauma, for extended prehospital Training courses (trainings) should take place within the framework of additional training of medical workers of health centers.
6.3. Skills and responsibilities of the head of the medical center of a general practitioner:
The head of the health center, the therapist is obliged to:
- Provide the Company's employees with qualified therapeutic assistance;
- Provide first aid for acute poisoning and diseases;
- To send, in the prescribed manner, for examination and inpatient treatment of patients in need of hospitalization, as well as with suspicion of an occupational disease to clarify the diagnosis;
- Monitor the health status of the Company's employees in order to prevent and timely detect general and occupational diseases;
- Participate in the formation of lists of employees for medical examinations, lists of contingents, consulting on the development of recommendations for identifying professional incompetence, filling in form 088 / u-06;
- To identify persons subject to clinical examination, followed by examination, dynamic observation and treatment;
- Conduct medical examination for long-term and often ill workers, as having additional risk factors for the development of diseases of the cardiovascular system;
- Arrange consultations with medical specialists of a narrow profile at the request of the Customer's employees;
- Monitor the outpatient and inpatient treatment of employees of 3,4 dispensary groups;
- Participate in the implementation of preventive measures to prevent and reduce morbidity with temporary disability, including the prevention of cardiovascular diseases;
- Organize and monitor the timely passage of preliminary and periodic medical examinations by employees, as well as psychiatric examination of the Customer's employees in conjunction with other services of the enterprise;
- Conduct sanitary and educational work, lectures, talks;
- Participate in preventive measures to prevent and reduce occupational diseases and injuries;
- To interact with the territorial department of Rospotrebnadzor, FBU RO "Center for Hygiene and Epidemiology in the Rostov Region and other organizations in the interests of the Customer;
- Maintain medical records and submit reports in accordance with established forms in accordance with the normative documentation of the Customer.
- Organize qualified physiotherapeutic assistance for the Company's employees;
- Develop a production control program for the Customer's health center in accordance with applicable law.
7 SUPPLIES WITH MEDICINES AND CONSUMABLES
The contractor, on his own and at his own expense, equips the health center with medicines and consumables.
The recommended minimum of drugs for a health center, the availability of which at any time must be provided by the Contractor:
