Comments: 1. More detailed information about the request is provided in the cover letter.
2. All forms of documents are obligatory for filling. To participate in the procurement procedure, you must complete and provide:
2.1 Map of the proposal and its annexes;
2.2 Calculating the calculation of additional costs for the mobilization/demobilization of equipment, machinery, equipment, delivery of personnel, food and staff accommodation;
2.3 Documents on the proposed equipment (certificate of Ownership, lease/lease agreements, PTS, PSM);
2.4 Documentation on employees (industrial safety (attestation in RTN), labor protection, fire and technical minimum, work with the GPM;
2.5 Letters of recommendation (in case of availability);
2.6 Package of constituent documents:
- Certificate of State registration (on the assignment of UAH);
- Certificate of tax registration;
- Letter of the Federal Service of State statistics on attribution of codes;
- Decision of the authorized body of the legal entity on the appointment of the Executive Body;
- Power of attorney for the person to whom the contract will be signed (if necessary);
- Charter of a legal entity (in the last edition with all amendments and additions);
- Extract from the unified State Register of legal entities issued by the Russian Federal Tax Administration (statute of limitations not exceeding 1 calendar month);
- Accounting (financial) reporting for the last reporting year, or for the last reporting period, if the counterparty makes interim accounting (financial) reporting, with the application of evidence of reporting to the Tax Inspectorate;
- Bank statement on the flow of funds for the last 3 months;
- Consent to the processing of personal data
- Copy of Passport, insurance Certificate of State pension insurance, filled-for individuals
3. Proposals will not be accepted without loaded documentation (completed forms of documents, annexes according to paragraph 2).
4. This procedure of request for quotation is not a contest and its holding is not regulated by articles 447-449 of the Civil Code of the Russian Federation;
5. This procedure is also not a public contest and is not regulated by articles 1057-1061 of the Civil Code of the Russian Federation;
6. Thus, this procedure of request for quotation does not impose on the customer the corresponding volume of civil obligations.
