2.12. The completed signed application form.
3. Reference list (experience of similar services in the past 3 years, with certified copies of the supporting documents: Act of the executed works, UPD with not less than 30 000 000 rub. without VAT)
4. Help on staff resources, completed in accordance with Annex 7 (to be completed separately for each lot). Help on staff resources should contain complete information about the staff involved in the provision of services, including specialist responsible for no-risk traffic.
5. List of contractor's vehicles, filled in the form of attachment 8.
To the list of vehicles must be accompanied:
5.1. Copy of passport for each TC TC
5.2. Copy of certificate about State registration of the VEHICLE for each VEHICLE
5.3. Copy of lease agreement with copies of acts of acceptance of the TC/copy of lease contract (in case if the participant is not the owner of the vehicle).
6. Written confirmation of the purchase of the principal under the drivers specified requirements:
- drivers have a valid medical certificate of standard pattern;
- neat appearance;
- strict observance of requirements of SDA RF compliance requirement from passengers.
10. additional (if possible) must make information/commercial proposal for the development of the concept of internal logistics (optimization, improvement of business processes).
