Albazino Resources LLC invites you to take part in the tender for the provision of services for a special assessment of working conditions in accordance with the terms of reference.

Place of provision of services:
1. Khabarovsk Territory, Polina Osipenko district, in the Amgun-Somnya interfluve, on the right bank of the Ernesty spring in its upper reaches, the site of the Albazin mining and processing plant (Coordinates of uch. Albazino Latitude 52 ° 51´ 17.7´´, Longitude 137 ° 53´ 2.6´´);
2. Khabarovsk Territory, district named after Polina Osipenko, Geological exploration site Talgiy (80 km from the production site. Albazino);
3. Khabarovsk Territory, Amursk, Mashinostroiteley Highway, 2 (50 km from Komsomolsk-on-Amur).

Terms of service provision:
1. Start date of services: 01.07.2021;
2. End date of services: 07/31/2021.

Stages of service provision:
1. stage - geological exploration section Talgiy (2 days);
2. stage - Albazino section (5 days);
3. stage - Amursk (1 day).

Scope of services:
It is carried out for employees according to the attached list given in Appendix 1 to the Terms of Reference (100 workplaces).

Scope of services:
A special assessment of working conditions is carried out in order to assess the working conditions of employees for compliance with state regulatory requirements for labor protection, to form a list of measures to improve the working conditions of employees and to establish the need to provide guarantees and compensation to employees.

Requirements for the Contractor:
Availability of an accreditation certificate for a special assessment of working conditions.
Carrying out SOUT in accordance with:
1. Federal Law No. 426-FZ of December 28, 2013 "On Special Assessment of Working Conditions";
2. Order of the Ministry of Labor of Russia dated January 24, 2014, No. 33n "On approval of the Methodology for conducting a special assessment of working conditions, the Classifier of harmful and (or) hazardous production factors, a report form for conducting a special assessment of working conditions and instructions for filling it out";
3. Order of the Ministry of Labor and the Ministry of Health of December 31, 2020 No. 988n / 1420n "On approval of the list of harmful and (or) hazardous production factors and services, in the performance of which mandatory preliminary medical examinations are carried out upon admission to the service and periodic medical examinations";
4. Regulatory legal acts in force at the time of the provision of services containing state regulatory requirements for labor protection and special assessment of working conditions.

Obligations of the Customer:
The Customer undertakes to provide the materials and documents necessary for the Executor to perform the services (services). The transfer of materials and documents is carried out according to the act of acceptance of the transfer of materials and documents, which is endorsed by the Parties. The customer is responsible for the authenticity and accuracy of the originals and copies of materials and documents provided by him;
Make payment within 30 (thirty) banking days after signing the acceptance certificate for the services performed. The amount of the advance payment is not more than 25% of the value of the contract or the cost of services;
Transport for the delivery of the contractor's personnel from the Albazino site to the Talgiy exploration site and back is organized by the Customer at its own expense.

Accommodation, meals, delivery of the Contractor's personnel to the Customer's facilities:
1. At the Customer's facilities (Albazino site, Talgiy geological exploration site) meals, accommodation and consumer services for the Contractor's personnel are organized by the Customer at its own expense (costs are not included in the calculation of the cost of services under the TOR);
2. At the Customer's site in Amursk, the Contractor independently provides its personnel with personal services (food, accommodation, consumer services). The costs must be included in the cost of services (with the provision of an appropriate cost estimate);
3. Delivery of personnel to the Customer's facilities (Albazino site, Talgiy geological exploration site) from the collection point in Komsomolsk-on-Amur and back is carried out by forces and at the expense of the Customer (expenses are not included in the calculation of the cost of services under the TOR);
4. Delivery of personnel from the location of the Contractor to the collection point (in the city of Khabarovsk or in the city of Komsomolsk-on-Amur) and back is provided by the Contractor on its own and at its own expense. The costs must be included in the cost of services (with the provision of an appropriate cost estimate);
5. The delivery of personnel from Komsomolsk-on-Amur to the Customer's facility in Amursk (after the provision of services at the Albazino site and the Talgiy exploration site) is organized by the Contractor at its own expense. The costs must be included in the cost of services (with the provision of an appropriate cost estimate);
6. Immediately before arriving at the Customer's facilities, the Contractor is quarantined for up to 3 to 5 days in a place specified by the Customer (in Khabarovsk or Komsomolsk-on-Amur). The place for quarantine is specified before mobilization (also before entering the quarantine, it is mandatory to provide a medical commission for each employee on the state of health, including the results of the current fluorographic study). For the period of quarantine, meals (three times a day), accommodation, medical care (examinations, thermometry, testing for COVID-19) of the Contractor's personnel is carried out at the expense of the Customer and are not included in the calculation of the cost of services under the TK. The Customer also ensures the delivery of the Contractor's personnel after personnel quarantine to the collection point in Komsomolsk-on-Amur.

We will be grateful to receive your commercial offer no later than 17:00 (Khabarovsk time) 05/11/2021 with the following attachments:
- commercial proposal on the official letterhead of the participant signed by an authorized person;
- proposal card (Appendix No. 2 to the letter);
- counterparty questionnaire (Appendix No. 3 to the letter) and reference list;
- scanned copies of permits required for the provision of services;
- constituent documents: charter (fully in the latest edition), certificate of the OGRN, certificate of TIN, decision of the founder on the appointment of the head, letter on assignment of codes (Rosstat);
- consent to the processing of personal data (in accordance with the form) (Appendix No. 4 to the letter).

This RFP procedure is not a tender and is not subject to Articles 447-449, Section 1 of the Civil Code of the Russian Federation;
This procedure is also not a public tender and is not regulated by Articles 1057-1061, Section 2 of the Civil Code of the Russian Federation;
Thus, this request for proposal procedure does not impose on the Customer the corresponding volume of civil and legal obligations.
