Comments: To participate in the procurement event, an organization must:
1. Fill out the participant's application form, providing in PDF format (scan with signature);
2. Fill out Annex 1 of the Participant's details, submitting in Excel format;
3. Fill out Appendix 2 Statement of Compliance (scan with signature). Attach a technical description of the equipment confirming the information provided in the Application;
4. Fill out Appendix 3 FinAnaliz_ rating 1, submitting in Excel format;
It is also necessary to prepare the following documents in separate PDF files:
- Extract from the register dated 2019;
- The current version of the statute with the current changes;
- Power of Attorney or other document authorizing a person to sign
documents on behalf of the organization;
- Certificate of registration with the tax authority;
- Certificate of entry in the register;
- The balance sheet or other documents characterizing the financial and economic activities of the organization, please provide for closed periods of the current year, as well as a report at the end of previous periods (maximum 3 years).
Electronic copies of documents must meet the following conditions:
• the participant should ensure that every scanned document is easy to read;
• each document must have a name (abbreviated) corresponding to its content.
The package of documents should be sent to e-mail addresses ALuzhnev@beeline.ru VKhudyan@beeline.ru no later than 16:45 on August 16, 2019 (in the subject line indicate "Participant name / Selection of suppliers of XFP / SFP / SFP / QSFP modules for the Russian Federation for 3 years. "
The total size of the letter should not exceed 10 MB (if necessary, you can send several letters or several volumes of one archive).
