To participate in prequalification, a supplier must:
1. Read the Privacy Statement;
Please note that your consent to participate in PJSC VimpelCom PJSC means you agree to the terms of the Privacy Statement, and you will be held liable for non-compliance with the Privacy Statement in accordance with applicable law.
2. Fill out the PROCUREMENT PARTICIPANT APPLICATION, providing it in PDF format (scanned with the signature of an authorized person).
3. Provide constituent documentation:
- Extract from the Unified State Register of Legal Entities, certified by the State Tax Inspection, dated not earlier than September 2020 (electronic signature is allowed);
- Charter;
- Order on the appointment of the General Director and the current Decision on the election / appointment of the head;
- copy of the certificate of state. registration (OGRN);
- copy of the TIN assignment certificate.
4. Fill in the FinAnaliz_rating file and attach supporting financial documents for the CLOSED reporting periods (2018-2019, 1 half of 2020) with a note from the tax office that the documents were accepted and that there were no errors (for electronic versions).
5. Get acquainted with the qualification requirements by submitting in PDF format a scan of the signed statement of compliance with the qualification requirements, the prequalification participant's questionnaire and scans of supporting documents.
Please, note! The archive with documents is password-protected, the password to the archive is specified in the file "Privacy Statement".
Electronic copies of documents must meet the following conditions:
1. The participant must ensure that each scanned document is easy to read;
2. Each scanned document must be saved in a separate file, while a document containing one page is saved in a separate file, and a document containing several pages and attachments must also be grouped into one file;
3. Each document should have a name (abbreviated) corresponding to its content.
PAYING ATTENTION!
The package of documents must be attached when responding to this prequalification no later than 15-00 (Moscow time) on October 9, 2020 or sent to the email address of the Procurement Department of the Western Region of PJSC VimpelCom: Tender_NWR@spb.beeline.ru.
