When submitting an application, it is required to provide completed annexes to the request, as well as in the absence of work experience with JSC Mikhailovsky GOK named after A.V. Varichev "- copies of constituent documents, a certificate of state registration with the assignment of the OGRN, a certificate of tax registration, financial statements (balance sheet + statement of financial results) for the last reporting year with a mark of the Federal Tax Service of Russia, a license to provide services for this activity (if availability).
2. In case of disagreement with the proposed form of the contract, when submitting an application, it is necessary to provide a protocol of disagreements.

The provided technical and commercial proposal must consist of two parts - technical (Appendix 1-4) and commercial (Appendix 5), certified by the signature of the head with a seal. In the technical part, the list and scope of work must be indicated and it is not allowed to indicate any information related to the commercial part (total cost, unit prices, payment terms, etc.). If necessary, separate documents of the technical and commercial parts of the TCH can duplicate each other, provided that such documents in the technical part of the TCH will be presented without specifying prices, and in the commercial part with prices. The technical and commercial proposal must strictly comply with the data indicated on the electronic site. If there is a discrepancy between the data in the attached TCH and the completed electronic form on the site, the electronic form takes precedence. In case of non-compliance with the above requirements, the TCH will be rejected. It is also necessary to consider the standard form of the contract (draft contract).
In case of providing false information, a potential service provider may be removed from participation in the Purchase, and if the inaccuracy of the information provided is revealed after the conclusion of the Agreement, such Agreement may be terminated.
