4. List and scope of work performed.
4.1. Implementation of the full complex of CMR according to the project ST13678, taking into account all the sets included in its composition of the projects -*reference and borrowed drawings:
• ST13678 sheet 1-6
• EL 001.13678 General data.$
• EL 002.13678 MH Power Supply mounting drawing.
• EL 002.13678 EZ Electrical supply scheme electric prince.
• EL 003.13678 Lighting. Mounting drawing.
• EL 004.13678 Earthing devices. Mounting drawing.
• SV 0010.13678 Installation of compressor.
• ТС010.13678 Heat supply.
4.2. This specification does not include works from the section of the project ST13678:
• Brickwork when expanding openings pos. D - 1, D - 3; $
• Brickwork of partitions and foundations under it;
• Removal and bookmark of window aperture note note 2, except for plaster internal and external;
• Dismantling and installation of door blocks approx. P2 D-1; D-3, except for plaster slopes:
• Installation of window block O-1.
• Dismantling of ventilation, water and gas pipes.
TC 010.13678 work is not performed.
4.3. The basis for calculating the cost of works are the volumes of work specified in the sections of the project СТ13678; EL 001.13678 EL 002.13678 MH; EL 002.13678 EZ; EL 003.13678; EL 004.13678; SV 0010.13678 with its components composition of subprojects and present TK. The Contractor independently develops the estimated documentation. The works provided for by the project and the present TK and not reflected in the estimates, cannot be considered additional.
If it is necessary to perform additional works not provided for in this task, the Contractor performs them on a separate task of the Customer with the compilation of estimates with indexes and coefficients, to be used in the estimate of the bulk of work of this task.$
The contractor primarily performs the foundation for the FM1 equipment.
The bookmark of the PR5 opening will be carried out within 3 working days after the installation of the measuring machine.
After laying the aperture PR5 dismantle the concrete ramp and restore the pavement with asphalt concrete, the volume of the dismantled ramp-3,6m3, the volume of the repaired sidewalk 3,6m2.
5. Supply of materials and construction machinery to perform this work.
5.1. Works are performed using the material and equipment of the Contractor, except for:
• Split systems — 2 pcs.
• Compressor — 1 pc.
5.2. Construction and lifting equipment, installation equipment, loading mechanisms, tools and all consumables for them (oxygen, propane, auxiliary materials), SIZ, electrowelding, gas-cutting and other equipment necessary for carrying out works is provided by the Contractor
5.3. The removal of garbage of all categories, to landfills of receiving industrial waste, is carried out by transport provided by the Customer, on the preliminary request of the Contractor. The costs of waste disposal of all categories, at landfills of receiving industrial waste, are borne by the Customer.
5.4. Loading of garbage is carried out by forces and mechanisms of the Contractor.
5.5. In case of reasonable comments to the personnel on the quality of performed works, violation of safety, as well as violations of administrative nature, at the request of specialists of the Customer the Contractor is obliged to replace these employees immediately.
6. Requirements to the Contractor
6.1. The contractor must have the necessary production and human resources to perform this work, be a member of the self-regulatory organization (SRO).
6.2. To clarify the conditions of performance of works, familiarization with the object it is desirable to arrive representatives of the Contractor to the Customer.
6.3. The Contractor in his commercial offer shall indicate the payment procedure and other conditions.
6.4. Certification of the head, engineering and performers on industrial safety and labor protection at the GCO according to Appendix №1.
6.5. Provision of H&P measures, safe methods of work at the site.
6.6. The Contractor shall provide the following documents to the Customer prior to the issuance of the Work Permit: copies of documents confirming the presence in the contractor organization of labor protection service and/or occupational safety specialist (order on appointment to a position in the contractor organization, certificate specialist of labor protection) or the existence of an agreement with an accredited organization in the field of labor protection services.
6.7. After completion of the works, the executor transfers to the customer the executive documentation according to RD-11-02-2006 dated 26.12.06 and other normative documents.
6.8. Guarantee for at least 12 months.
6.9. Workers employed for the performance of work must be trained in accordance with the nature of the work performed, with a reference to the special certificate of completion of training and handover $ exams in the attestation commissions.
7. Safety of work
When performing works, the contractor shall comply with the requirements of the following rules:
- rules on labor protection during operation of electrical installations;
- rules of technical operation of electrical installations of consumers;
- safety rules of hazardous production facilities where lifting facilities are used;
- rules for the installation of electrical installations;
- fire safety regulations;
- rules on labor protection when working at the height of the POT RM-012-200;
- environmental, sanitary-hygienic and other regulations in force on the territory of the Russian Federation and to ensure safe working conditions for the life and health of people.
In order to perform the work, the contractor must undergo an introductory instruction. All works performed on the territory of JSC "EPM-NEZ" shall be carried out in accordance with the provisions "On the application of permits in the production of high-risk works at JSC "EPM-NEZ"" ;.
Appendix #1
The list of documents submitted to the management of occupational safety and security for the registration of work permits by contractors:
1. Copy of the Certificate (SRO) for the right to perform this type of work.
2. A copy of the instruction on labor protection from the briefing journal.
3. ERP, RDP or RTI (if required) .$
4. Copy of the report of certification of workers on labor protection.
5. Copies of certificates of certification of labor protection and general requirements of industrial safety. When performing works on gas, on vessels working under pressure, using PS, in electrical installations, carrying out fire works - copies of certificates for each type of work.
6. Medical admission of staff.
7. Copies of certificates of gas-electric welders, slingers, drivers of cranes, lifts (towers). For gas-electrowelders - copies of protocols of certification on fire-technical minimum.
8. The Contractor shall provide the following documents to the Customer prior to the issuance of the Work Permit: copies of documents confirming the presence in the contractor organization of labor protection service and/or occupational safety specialist (order on appointment to a position in the contractor organization, certificate specialist of labor protection) or the existence of an agreement with an accredited organization in the field of labor protection services. All employees of the contractor, including supervisors, are required to undergo induction training in the OTPB and the Civil Service (to have a document certifying identity and age) When performing the work, all employees must have certified clothing, footwear and other personal protective equipment necessary to perform this type of work.
9. In case of discrepancies of information in the submitted documents to the requirements of regulatory and legal acts, the Customer has the right to initiate audit by the Health and Safety Service in matters of Occupational Safety and Health and Safety. Industrial safety in the Contractor's organization.
Note: In the event of expiry of any document, the permit is not issued until the new document is submitted.
