1) A commercial offer, taking into account all taxes and fees, as well as overhead and transportation costs necessary for the implementation of the project in accordance with the stated Terms of Reference (in PDF format on the letterhead of the organization with the seal and signature of an authorized person);
2) Completed Appendix No. 2 (Application for Consent) in PDF format on the letterhead of the organization with the seal and signature of the authorized person;
3) Completed Appendix No. 4 (Declaration of Compliance with Customer requirements) in PDF format on the letterhead of the organization with the seal and signature of an authorized person;
4) A completed Participant Questionnaire in the form of Appendix No. 6 (in Word and PDF format with the signature of the authorized person and seal) with all the appendices required in the questionnaire;
5) A presentation describing the approach to the project, including:
• Company information
• Approach to project implementation
• Scope of services within the project
• General project schedule
• Organizational structure of the project team and their roles
• Description of the team with a summary of key experts proposed for the project (project manager, main consultants);
6) A certificate of completed projects in the form of Appendix No. 8;
