11. The purpose of the work:
Obligatory periodic medical examination of workers associated with harmful and (or) dangerous working conditions in accordance with Article 212, Article 213 of the Labor Code of the Russian Federation, Order of the Ministry of Health and Social Development of the Russian Federation dated April 12, 2011 No. 302-N "On approval lists of harmful and (or) hazardous production factors and work, during which mandatory preliminary and periodic medical examinations (examinations) are conducted, and the procedure for conducting mandatory preliminary and periodic medical examinations Mothra (surveys) workers engaged in cha-heavy work and work with harmful and (or) hazardous working conditions, "getting me-ditsinskih conclusions about the suitability of the professional staff.
12. Time, place for examination and number of subjects:
12.1. In 2018, 1,057 people should undergo a periodic medical examination, and 384 people should additionally have a fluorography.
12.2. Duration: August-September 2018, location: Novocherkassk, as agreed with the Customer.

14. Requirements for the Contractor and the conditions for the periodic medical examination:
14.1. The contractor in his commercial proposal indicates the procedure for payment of work, the cost of the proposal with VAT / without VAT, other conditions;
14.2. The Contractor provides to the email address specified in this Task, documents confirming the qualifications of medical personnel, allowing to conduct an examination of professional suitability (copies of licenses, certificates of doctors);
14.3. The contractor uses a fluorographic apparatus with documents for conducting fluorographic studies; other medical equipment (ECG, ultrasound, etc.), has available the necessary supplies and tools in a sufficient amount;
14.4. The Contractor submits documents for the equipment used for conducting the PMO (technical passport, certificate, verification certificates), certified by seals and signatures, in color copies in electronic form in PDF format to the email address specified in this Task;
14.5. The contractor should have the right to conduct an examination of professional suitability and an examination of the connection of the disease with the profession.
14.6. Based on the occupational hazards or occupations indicated in the contingent of professions, the Contractor’s medical commission determines the need for appropriate medical specialists to participate in periodic examinations, as well as the types and volumes of necessary laboratory and functional studies.
- A periodic inspection is completed if the employee is examined by all specialist doctors, as well as the full scope of laboratory and functional studies is provided for in the Factor List or the Work List.
- At the end of the employee’s periodic examination by a medical organization, a medical report is issued.
- Based on the results of the periodic examination in the prescribed manner, the employee’s membership in one of the dispensary groups is determined in accordance with applicable regulatory legal acts, followed by the issuance of recommendations on the prevention of diseases, including occupational diseases, in the medical card and health passport, and if any medical indications - for further observation, treatment and rehabilitation.
5.7. Based on the results of the examinations, the Contractor, not later than 30 days after the completion of the periodic medical examination, summarizes the results of the periodic periodic examinations of employees and draws up a final act.
- The final act is drawn up in four copies, which are sent by the Executive within 5 working days from the date of approval of the act to the employer, to the center of occupational pathology of the subject of the Russian Federation, the territorial body of the federal executive body authorized to carry out state control and supervision in the field of ensuring sanitary and epidemiological welfare of the population.
- The issued medical documentation based on the results of the periodic medical examination - employee health passports and the final act are provided to the Customer no later than 30 days after the completion of the periodic examination.
6. Other conditions
The Commission reserves the right to choose any organization at its discretion.
Comments: The participant who participated in the procedure (made an offer), before the deadline for accepting applications, must upload the following documents to the system:
1. Charter (with all additions and changes, if any).
2. Certificate of state registration.
3. Certificate of tax registration.
4. Extract from the register.
5. The balance sheet as of the last reporting date with a mark of the tax authority on acceptance.

10. Requirements for the Contractor:
