4. Purpose of work:
Carrying out work on the lining of the new cover of the re-firing furnace No. 3, inv. No. 6469
5. The list and scope of work performed: according to the drawing ПЧ 006416 СБ
7.1. Work on the lining of the roof cover of the re-firing furnace No. 3 TsPOiP inv. No. 6469 is made from the material of the Customer. (Fireclay heat-insulating refractory products SL-23 STO 05802307-0-002-2014 and heat-resistant mastic MT-1)
Equipment according to PPR, tools and equipment necessary for carrying out the entire complex of works - provided by the Contractor.
7.2. Work in a working enterprise. The enterprise operates around the clock.
7.3. The impact of harmful factors is possible: dustiness, gas pollution, work at height.
7.4. Development of ERP, PPR and flow charts for all necessary work - no more than 10 calendar days after the announcement of the winner of the competitive procedure.
7.5. The contractor must have the necessary production and human qualified resources to carry out this work. Have a certification of a manager, engineering technician and workers in industrial safety, as well as a medical clearance
7.6. SRO availability for these types of work.
7.7. Availability of machines and mechanisms for the entire range of works.
7.8. The Contractor's personnel must have and use personal protective equipment.
7.9. In case of comments to the staff on the quality of work performed, violation of technology
security, as well as violations of an administrative nature, at the request of the Customer's specialists, the Contractor is obliged to immediately replace these employees.
7.10. Work performed not of high quality must be redone at the expense of the Contractor and handed over to the Customer.
7.11. The placement of workers employed to carry out this work is carried out by the Contractor.
The customer, if possible, can carry out the placement of workers on a contractual basis.
7.12. Development and approval of a linear schedule for the repair of the furnace - within a period not exceeding
10 calendar days after the announcement of the winner of the competitive procedure.
8. Payment terms: monthly, within 30 (thirty) banking days after delivery
results of work, provided that the work is done properly and within the agreed time frame. The work is considered completed after the Parties have signed the Act of acceptance of the work performed (form KS-2) and the Certificate of the cost of work performed and costs (form KS-3).
9. Terms of work: Two months (approximately from 05/15/2020 to 08/31/2020). The beginning of work is agreed with the Customer additionally
10. Warranty obligations for the work performed: 24 months from the date of completion of the work.
11. The furnace is repaired in the existing workshop. The repair schedule is agreed with the shop (TsPOiP) and the plant's services (UM, EU, UA, UE).
12. The list of documents provided to OTPB EGOiChS for registration of work permits for work by contractors:
1. Copies of the Certificate (SRO) for the right to perform this type of work.
2. Copies of the OSH briefing from the briefing journal.
3. POR, PPR or RTI.
4. Copies of the protocol of certification of workers for labor protection.
5. Copies of certificates of attestation of engineers and technicians for labor protection and general industrial safety requirements. When performing work on gas, on vessels operating under pressure, using PMG, elevators (towers), in electrical installations, conducting hot work - copies of certificates for each type of work.
6. Medical clearance of personnel.
7. Copies of certificates of gas electric welders, slingers, crane drivers, hoists (towers). For gas-electric welders - copies of certification protocols for fire-technical minimum.
Before the start of registration of the order - admission, all employees of the contractor, including managers, are required to undergo an introductory briefing at the OTPB EGOiChS (with you have a document confirming your identity and age). When performing work, all employees must have certified clothing, footwear and other personal protective equipment necessary to perform this type of work.
In the event of the expiration of any document, the work permit is not granted until a new document is submitted.
13. Note:
- Delivery of material from the warehouses of OJSC "EPM-NEZ" to the place of work, removal of waste (not slagged refractory bricks, scrap metal, etc.) to storage sites located on the territory of OJSC "EPM-NEZ" is carried out by transport provided by the Customer, according to preliminary the Contractor's request.
- Removal of garbage of all categories to landfills for receiving industrial waste located outside the territory of EPM-NEZ JSC (garbage removal at a distance of 7 km to 30 km) is carried out by transport provided by the Customer, upon the preliminary request of the Contractor.
- The expenses for the disposal of garbage of all categories at the landfills for receiving industrial waste located outside the territory of EPM-NEZ OJSC are at the expense of the Customer.
- Delivery of tooling in accordance with PPR, tools and equipment required for the entire range of works from the Contractor's warehouses to the work site on the territory of EPM-NEZ JSC is carried out by the Contractor's transport.

- a copy of the instruction on labor protection from the instruction log;
- POR or PPR or RTI;
- copy of the protocol of certification of workers for labor protection;
- copies of certificates of attestation of engineers and technicians for labor protection and general industrial safety requirements. When performing work on gas, on vessels operating under pressure, using PMG, in electrical installations, during hot work - copies of certificates for each type of work;
- medical clearance of personnel - copies of certificates of gas-electric welders, slingers, crane drivers, hoists (towers). For gas-electric welders, copies of the certification protocols for PTM.
In case of revealing inconsistencies of the information in the submitted documents with the requirements of regulatory legal acts, the Customer has the right to initiate an audit by the Health and Safety Service in matters of Labor Protection and Industrial Safety in the Contractor's organization.
The Contractor notifies the Customer about the fulfillment of all the specified conditions in writing with the attachment of all supporting documents. Failure to comply with the terms of this clause by the Contractor gives the Customer the right to prevent the Contractor from performing the work, the delay in the performance of the work in this case is the Contractor's fault
