4. Content of work on overhaul
4.1. Electricity supply works are carried out from the material of the contractor for the project EL 002 13615 ABK Block 4. Dining room. Power supply. (in part - only power supply from TP 21 2.s. 0.4 kV feeder No. 16). In addition to the project specification, it is necessary to purchase and install four connecting couplings 4ПСТ-1-70 / 120.
4.2. Lifting mechanisms for dismantling and installation of equipment, tools,
mechanisms for loading and unloading are provided by the Contractor.
4.3. Motor vehicles for the removal of garbage and other waste are provided by the Contractor.
4.5. Electric welding, gas cutting equipment, consumables
provided by the Contractor.
5. Requirements for the Contractor
5.1. The contractor must have the necessary production and human resources
To perform this work, be a member of a self-regulatory organization (SRO).
5.2. To clarify the conditions of work, familiarization with the object is desirable
arrival of representatives of the Contractor to the Customer.
5.3. The contractor in his commercial proposal indicates the payment procedure
and other conditions.
5.4. Certification of the head, engineers and industrial safety performers and
labor protection at the OSE in accordance with Appendix No. 1.
5.5. Provision of health and safety measures, safe methods of production work at the facility.
5.6. After completion of work, the contractor transfers the executive
documentation in accordance with RD-11-02-2006 of 12.26.06 and other regulatory
documents.
5.7. Guaranteeing at least 18 months.
Workers involved in the repair of electricity supply to the dining room must have professional training corresponding to the nature of the work with a mark in the special certificate of training and exams in the certification commissions.
6. Work safety
When performing repairs on the electricity supply to the dining room, the contractor must comply with the following rules:
- labor protection rules for the operation of electrical installations;
- rules for the technical operation of electrical installations of consumers;
- safety rules for hazardous production facilities that use lifting facilities;
- electrical installation rules;
- fire safety regulations;
- rules on labor protection when working at height POT RM-012-200;
- the rules of environmental, sanitary and hygienic and other norms in force in the Russian Federation and to ensure safe working conditions for human life and health.
To perform the work, the contractor must undergo introductory training. All work performed on the territory of EPM-NEZ JSC must be carried out in accordance with the provision "On the use of tolerance orders in the work of increased danger at EPM-NEZ JSC".
Appendix No. 1
The list of documents provided to the Department of Health and Safety for the execution of work orders - tolerances for the performance of work by contractors:
1. A copy of the labor safety briefing from the briefing magazine.
2. POR, PPR or RTI (if required).
3. Copy of the protocol of certification of workers for labor protection.
4. Copies of certificates of certification of labor protection engineering and general industrial safety requirements. When performing work on gas, on vessels operating under pressure, using substation, in electrical installations, conducting hot work - copies of certificates for each type of work.
5. Medical admission staff.
6. Copies of certificates of gas electric welders, slingers, crane drivers, hoists (towers). For gas and electric welders - copies of certification protocols for fire-technical minimum.
7. Prior to completing the work permit, the Contractor must provide the Customer with the following documents: copies of documents confirming the existence of a labor protection service and / or labor protection specialist in the contracting organization (order for appointment to the position in the contracting organization, certification of a labor protection specialist) or the existence of a contract with an accredited organization in the field of providing labor protection services. All employees of the contracting organization, including managers, are required to undergo introductory training in the Department of Occupational Health and Safety (have a document proving their identity and age). When performing work, all employees must have certified clothing, shoes and other personal protective equipment necessary to perform this type of work
8. In case of discrepancies in the information presented in the documents to the requirements of regulatory legal acts, the Customer has the right to initiate an audit by the OH&S service on labor protection and industrial safety issues in the Contractor’s organization.
Note: In the event of the expiration of any document, the admission order is not issued before the submission of a new document.
Comments: The participant who participated in the procedure (made an offer), before the deadline for accepting applications, is obligated to upload the following documents to the system:
1. Articles of Association (with all the amendments, if any).
2. Certificate of the state registration.
3. Certificate of tax registration.
4. Extract from USRLE.
5. Balance sheet as of the last reporting date with tax authority stamp of acceptance.
