3. The title of the works:
3.1. Production of the D6248 press (c.6300 tn) pressing and main plunger rigging works located in the workshop of DCG LLC (c. Chelyabinsk), their transportation to Instromet LLC. Naberezhnye Chelny for repair. The overall dimensions of the extruding plunger are: length 5200 mm, greatest diameter 1680 mm, weight 33 tn., main plunger dimensions: length 5720 mm, greatest diameter 1680 mm, weight 52.5 tn.
PS: Work is done in a shop in a stopped production facility in which lighting (electricity) is connected and there is an active bridge crane Q=30 tn.
4. Purpose of the work:
4.1. Transport of press units to Instromet LLC (c. Naberezhnye Chelny).
4.2. Carrier manufactures the necessary tooling for transport at cost
works
5. Documentation:
Attached plunger drawing 28-402, 22-003A
6. Performer Requirement:
• The executor must have the necessary production and human resources to carry out the work;
• Production of work with possible exposure to harmful production factors: dust, gas, noise;
• Production of work in the tight conditions of a stopped shop,
• The Contractor is required to provide the following documents to the Customer prior to the clearance of the work permit:
- a copy of the certificate (SRO) for the right to perform this type of work;
- copies of documents confirming a occupational health service and/or occupational health professional (contract appointment order, certificate occupational health professional) or having a contract with an accredited organization in the field of occupational health services;
- a copy of the occupational health briefing from the briefing log;
- ERP or PPR or RTI;
- a copy of the Occupational Safety Workers' Certification Protocol;
- copies of ITR certification certificates for occupational health and general industrial safety requirements;
- medical clearance of staff.
The Contractor shall notify the Customer in writing with the appendix of all supporting documents. Failure to comply with the terms of this paragraph by the Contractor entitles the Customer to prevent Contractor from performing the work, the delay in this case is the Contractor's fault.

10. The Contractor is required to provide the following documents to the Customer prior to the clearance of the work permit:

- copies of documents confirming a occupational health service and/or occupational health professional (contract appointment order, certificate occupational health professional) or having a contract with an accredited organization in the field of occupational health services;
- a copy of the occupational health briefing from the briefing log;

- a copy of the Occupational Safety Workers' Certification Protocol;
- copies of ITR certification certificates for occupational health and general industrial safety requirements. When performing gas, pressure receptacles, GPM, electrical installations, fire, copies of the certificates for each type of work;
- staff medical clearance;
- copies of the certificates of gas welders, rafers, cranes machinists, hoists (towers). For electric gas welders, copies of PTM certification protocols.
In case of discrepancies in the submitted documents, the Customer is entitled to initiate an audit by the OT and PB in occupational safety matters and Industrial safety in the Contractor organization.
