3. A copy of the certificate of registration with the tax authority.
4. A copy of the notice of entry in the register of organizations conducting a special assessment of working conditions.
5. A copy of the accreditation certificate and the area of accreditation of the testing laboratory.
6. Extract from the register / EGRIP.
