2. Purpose of the works
Ensuring reliable functioning of the main equipment (lines of automatic processing of electrode products "Wassmer" and RL30, Mixers "Airich").
3. Reason for execution of works and deadlines:
Measures of maintenance of UA for 2020 Term of work: 2 quarter 2020 and until termination of the contract.
4. Scope of work performed
Currently, JSC "EPM — NEZ" has 7 modules - Rittal 3310740, 2 modules - Rittal 3332540, 6 modules - 3387540.
The Customer shall notify by e-mail about the necessary repair of the Contractor. The Contractor on his road transport takes from the customer modules for repair or maintenance. Performs repairs on its territory with the use of its forces and means.
The repair specification is drawn up and agreed with the representative of the Customer. The cost of repair, transportation and maintenance work is carried out at the prices of the price list, which is an annex to the contract. The price list should have the following composition of works: in TK
and shall be attached to the commercial offer for the performance of the work. In the commercial offer it is necessary to specify where the Contractor already produces these types of works, as well as the order of performance of works (in our territory or at home).
The cost of spare parts is included in the repair specification.
If the Contractor has the certification of personnel on industrial safety, it is possible to organize works on the territory of JSC "EPM-NEZ", but with mandatory compliance with the requirements of the TZ on occupational health and safety PB
5. Procedure for control and acceptance of works
The results of works are issued in the form of acts of performed works after presentation of the equipment to the Customer and verification of its operability.
6. BFDS requirements
When performing works on the territory of the Customer, the Contractor (contractor) provides the list of documents necessary for the registration of orders - permits for the work of contractors .
The Contractor shall provide the following documents to the Customer prior to the issuance of the Work Permit:
• Copies of documents confirming the presence in the contractor organization of labor protection service and/or occupational safety specialist (order on appointment to a position in the contractor organization, certificate specialist of labor protection) or the existence of an agreement with an accredited organization in the field of labor protection services;
• Copy of the Certificate (SRO) for the right to perform this type of work;
• Copies of certificates of certification of labor protection and general requirements of industrial safety. When carrying out work on gas, on pressure vessels, using GPM, lifts (towers), in electrical installations, carrying out fire works - copies of certificates for each type of work;
• Medical admission of staff;
• Copies of certificates of gas-electric welders, slingers, drivers of cranes, lifts (towers). For gas-electric welders - copies of fire and technical minimum certification protocols
• Prior to the commencement of the work permit, all employees of the contractor, including managers, are required to undergo an introductory instruction at the OTIPS (with them a document confirming their identity and age). All workers must have certified clothing, footwear and other personal protective equipment necessary to perform this type of work.
• Note: If any document expires, the permit will not be issued until the new document is submitted.
In case of discrepancies of information in the submitted documents to the requirements of regulatory and legal acts, the Customer has the right to initiate audit by the Health and Safety Service in matters of Occupational Safety and Health and Safety. Industrial safety in the Contractor's organization.
The Contractor shall notify the Customer in writing of all these conditions and shall be accompanied by all supporting documents. Failure to comply with the provisions of this clause by the Contractor entitles the Customer to prevent the Contractor from performing the works, in which case the delay in performance of the works is the fault of the Contractor.
7. Conditions of work
7.1 Work in an existing enterprise.
7.2. The equipment shall be operated in conditions of increased dust by carbon graphite dust.
Comments: The participant who took part in the procedure (who made an offer) must upload the following documents to the system before the deadline for accepting applications:
1. Articles of Association (with all the amendments, if any).
2. Certificate of the state registration.
3. Certificate of tax registration.
4. Extract from USRLE.
5. Balance sheet as of the last reporting date with tax authority stamp of acceptance.

10. Requirements for the performer:
