Comments:
LLC "Resources of Albazino" invites you to take part in the competition for the performance of repair work at the Processing plant, the supply of goods and materials, the implementation of SMR at the facilities:
1. HC 0002471 The main building of the processing plant;
2. HK 0002472 Large crushing housing with conveyor trestles No. 1-3.

Place of service provision:
Khabarovsk Territory, Polina Osipenko district, the site of the Albazinsky mining and Processing Plant.

Terms of work production:
Start of work: 16.08.2021;
Completion of works: 10.10.2021.
The start of work is allowed by a letter of guarantee from the Customer until the conclusion of the contract.
Before mobilization to the customer's site for work, it is necessary to undergo a quarantine, lasting 3 days (depending on the epidemiological situation). The date of arrival for quarantine is specified immediately before arrival.

Scope of work:
HC 0002471 The main building of the processing plant:
Repair of fire-resistant coating of load-bearing metal structures.
HK 0002472 Large crushing body with conveyor trestles No. 1-3:
Replacement of damaged elements of one of the racks along the tension axis, as well as 3 columns of the overpass.

Requirements for the production of SMR:
1. The Contractor's employees must be provided with personal protective equipment, protective clothing and special shoes.
2. Organization of SMR production within the established time frame with mandatory operational quality control of work at the work site - in accordance with the requirements of the regulatory documentation of the Russian Federation for the work performed.
- Provide laboratory quality control of the concrete mixture. During the work, all laboratory tests provided for by the construction norms and rules of the Russian Federation (concrete testing for strength, frost resistance, water resistance, etc.) must be carried out.
- In the Contractor's area of responsibility, ensure the specified quality of the concrete mixture in accordance with the requirements of GOST 7473-2010 "Concrete mixtures. Technical conditions".
- Delivery of finished samples (cubes) to the research site by and at the expense of the Contractor.
- Ensure quality control of welded joints: external inspection in the amount of 100 % of the seams; non-destructive testing methods (ultrasound) in the amount of 0.5 % of the length of the seams.
3. The Contractor's ITR, appointed by the Contractor's order, must be instructed by the Customer on OT and PB before the start of repair work, familiarized with the SUOT and PB of JSC Polymetal UK;
4. When performing work, the Contractor is obliged to comply with the requirements of labor protection and industrial safety, environmental protection, fire safety, to comply with administrative acts and internal regulations of LLC Resources Albazino.
5. When performing repair work, the presence of certified IT specialists of the Contractor who have a higher construction education or a special construction education and work experience in the specialty of at least 5 years is mandatory for the general management of the repair work carried out and the Contractor's employees.
6. The Contractor is obliged to maintain and provide the Customer with the executive documentation (general work log, certificates of inspection of hidden works, etc.) during the execution of works and after their completion, the current executive documentation is provided by the Contractor for verification to the Customer at the first request.

Minimum necessary requirements for the Contractor's machines and mechanisms (with an indication of the list and quantity for each type of equipment):
1. Truck crane, g/p up to 5 tons - 1 unit.;
2. Dump truck g/p up to 5 tons - 1 unit. (for the transportation of goods and materials, the removal of construction debris)
3. Concrete mixer - 1 unit.;
4. Welding equipment -1 units.;
5. Construction, inventory forests, up to 14 m high - 1 kmp.;
6. Equipment for drilling wells with a depth of 1m, Ø up to 150 mm - 1 unit.;
7. Manual and auxiliary tools for all types of work (puncher, USM, drill, screwdriver, welding machine, manual ramming, tools and accessories for painting works, including scaffolds, ladders, extension cords, for electrical work, etc.) - the number is determined by the composition of the team. All employees should be provided with the necessary tools.

Basic requirements for the contractor's personnel (number / qualification / working hours):
1. As part of the team, it is mandatory to have employees with certificates of admission to work at height, slingers ' certificates, electrical safety certificates (not lower than group II), welding production specialists must be certified (the presence of a valid certification certificate is mandatory) by the NAKS 1st level (group of technical devices-SC), and it is also necessary to have the conclusion of the medical commission on admission to work in the Far North and equated to them for all employees of the Contractor.
2. Control over the production of works is carried out by a person appointed by an order with the appropriate certificates (geodetic control, construction control), a specialist must be included in the LDC, have a NAKS certification of the 2nd level, an electrical safety admission group of at least IV, and also have the necessary equipment for monitoring.
3. As part of the ITR, it is mandatory to have a VET engineer at the facility with the presence of office equipment (laptop, printer, scanner, etc.), stationery and consumables necessary for the execution of executive documentation.
4. The mode of production of works is determined by the internal Order of the Contractor, taking into account the requirements for the production of works on the part of the Customer.

Materials provided by the Customer:
1. The customer provides the goods and materials, according to the list in Appendix No. 4 to the TOR;
In case of an increase in the volume of work on the preparation of concrete and mortar, the supply of inert remains with the Customer.
2. The goods and materials are transferred to the contractor at the AGOC warehouse site upon request;
3. Delivery of goods and materials from the warehouse site to the object is the responsibility of the Contractor.

Requirements for the availability of the contractor's permits:
1. In accordance with the requirements of the Urban Planning Code of the Russian Federation-the provision of an extract from the Register of SRO-S members with the right to conduct work on OPO objects is mandatory
2. The presence of an accredited construction laboratory at the contractor (or involvement under the contract).

The remaining requirements are specified in the invitation letter in the procurement documentation.

- a commercial offer on the official letterhead of the participant signed by an authorized person with information on the approval of the basic draft of the contract without disagreements, or the provision of an appropriate protocol of disagreements;
- completed offer card (Appendix No. 3 to the letter);
- completed calculation of the contract price (Appendix No. 2 to the TOR);
- the completed Questionnaire of the counterparty (Appendix No. 4 to the letter) and the reference list;
- scanned copies of the permits required to perform the work;
- scanned copies of documents confirming the availability of the necessary equipment, equipment;
- the charter (in the latest version with all amendments and additions);
- certificate of state registration (on assignment of the OGRN);
- certificate of tax registration;
- decision of the authorized management body of a legal entity on the appointment of an executive body;
- if necessary – a power of attorney for the person who signed the contract;
- letter from the Federal State Statistics Service on the assignment of codes;
- extract from the Unified State Register of Legal Entities;
- consent to the processing of personal data (in accordance with the form) (Appendix No. 5 to the letter).

This request for proposal procedure is not a tender and its conduct is not regulated by Articles 447-449 Part 1 of the Civil Code of the Russian Federation;
This procedure is also not a public competition and is not regulated by Articles 1057-1061 Part 2 of the Civil Code of the Russian Federation;
Thus, this procedure for requesting an offer does not impose an appropriate amount of civil obligations on the Customer.
