1) Information about the organization conducting a special assessment of working conditions, with the application of copies of documents confirming its compliance with the established by the federal law of 28.12.2013 № 426-FZ "On special assessment of working conditions" requirements;
2) A list of workplaces where a special assessment of working conditions was carried out, indicating harmful and (or) dangerous factors that are identified in these workplaces;
3) Maps of special assessment of working conditions, containing information about the organization established by the expert, conducting a special assessment of working conditions, class (subclass) of working conditions at specific workplaces;
4) Protocols of conducting research (tests) and measurements of identified harmful and (or) hazardous production factors;
5) Summary statement of the special assessment of working conditions;
6) The list of measures on improvement of conditions and protection of work of workers, on workplaces of which the special estimation of working conditions was carried out;
7) Expert opinion of an organization conducting a special assessment of working conditions;
8) Act of work performed.
The act of performed work on special assessment of working conditions is signed by the customer after receiving the above documentation from the executor. The ownership of the result of the executed works passes to the customer from the moment of signing by the parties of the work acceptance certificate.
Maps of special assessment of working conditions, protocols of measurements and estimations, drafts of final documents under the procedure of special assessment of working conditions shall be issued in accordance with the federal law of 28.12.2013 № 426-FZ "on special assessment of conditions Labour ".
