Reason for: Appendix to the procedure for conducting periodic medical examinations (surveys) of workers engaged in heavy work and work with harmful and (or) dangerous working conditions, approved. Ministry of Health and Social Development of the Russian Federation of April 12, 2011 N 302n
1. Application: KP with stamp and signature.
2. A license for medical activity, including work (services) on medical examinations (preliminary, periodical), examination of professional suitability, examination of the relationship of the disease to the profession, medical examination for the presence of medical contraindications to driving a vehicle, psychiatric and drug treatment.
3. Owned by the contractor of medical equipment necessary for conducting a comprehensive medical examination according to the list of necessary services.
4. The advantage will be the presence of a psychiatrist and a narcologist in a medical institution, as well as the presence of an X-ray machine for fluorography.
5. Prior to the moment of opening the envelopes, it is necessary to attach the proposals of the applicant and documents according to the questionnaire simultaneously with the submission of the proposal and the file as a scanned copy of their commercial proposal signed by an authorized person and certified by the stamp of the organization participating in this offer. If the participant fails to fulfill or improperly performs this procedure, the organizer of the procedure reserves the right not to consider the proposal of this participant.
The validity period of the suppliers' offer is at least 30 days.
Note: This request for proposals is not a notice of the tender (public tender) or a public offer and is not regulated by Art. 447 - 449 h. 1 of the Civil Code, art. 1057-1061 h. 2 of the Civil Code of the Russian Federation. LLC GSI SNEMA reserves the right not to inform the participants of the procedure of its results, except for the winner, to cancel the procedure or its results, as well as to exclude any of the participants in the procedure without giving a reason.
