2. Copy of the certificate of State registration of a legal entity (index of document 2-su);

4. Extract from the unified State Register of legal entities not older than 15 calendar days before the date of submission (Index of document 4-su);
5. Copy of okved certificate (information letter on statistics) (Index of document 5-su);
6. A copy of the certificate from the tax authorities on the tax arrears in the budgets of all levels (index of the document 6-su);
7. Copies of balance sheets together with profit and loss statements with the tax inspection mark for three years of activity and for the last reporting period (index of document 7-su);
8. Copies of documents confirming the election and appointment of a person acting on behalf of a legal entity without a power of attorney (hereinafter – the Head) (Order of appointment and minutes of the meeting of the authorized bodies on election (appointment)) (Index of the document 8-su);
