6. Appendix No. 5 to the Notice - Appendix to the Terms of Reference.

Please provide an Application for participation in the established form (Appendix No. 2) and all necessary documentation in accordance with the rules of the procurement procedure (Appendix No. 3).
I draw your attention to the fact that violations identified during the filing of the Application may serve as a basis for rejection of such a qualification application.
This FFP is a continuation of FFP 1450770 due to changes in the quantity of equipment.
