THE Bidder'S REFUSAL OF HIS CONFIRMED OFFER, as well as VIOLATION OF THE CONDITIONS set out in the tender procedure and the signed contract, LEAD TO THE DISQUALIFICATION OF THE Bidder (termination of cooperation with CB) FOR A PERIOD OF AT LEAST 1 YEAR.

IMPORTANT
It is necessary to provide (attach on the site) the following list of documents:
1. Completed questionnaire of the counterparty company (Appendix No. 3 "Counterparty Questionnaire")
2. Company card, certified by seal and signature
3. A copy of the Company's Charter (or amendments thereto) /1str., the main provisions, the composition of the Bidders, which body the executive body is elected, the term of office of the executive body, the size of the authorized capital, the last sheet (negotiable, with the registration number of the registering body)
4. Copy of the decision to create the organization
5. Copy of the certificate of state registration (for registration before 2017)
6. Copy of the tax registration certificate
7. The decision (protocol) on the appointment and/or extension of the powers of the General Director, management company, etc. – current as of the date of the agreement. If the contract is signed by a representative by proxy - a copy of the corresponding power of attorney
8. A copy of an extract from the Unified State Register of Legal Entities/ USRIP (for sole proprietors), with a validity period not later than 1 (one) month, from the date of its issuance
9. Lease agreement or certificate of ownership for the office (at the address of the actual location) and at the place of provision of services (if there are several objects, contracts are provided for all objects)
10. Balance sheet as of the last reporting date
11. Income tax or tax return for the previous year and as of the last reporting date with a note from the tax authority on acceptance
12. VAT declarations for the last 4 quarters with a mark of the tax authority on acceptance
13. RSV as of the last reporting date
14. Personalized information about individuals for the previous month (title page and data sheets for employees providing services can be with private personal data)

17. A certificate from the tax authority on the presence/absence of debt to the budget (original), due to the deadline for receipt, the document can be provided after the competitive bidding process is completed

- personal income tax declarations F-6 (the last two tax periods), USN (the last two tax periods)
If it is necessary to carry out additional verification activities, additional documents may be requested (with preliminary justification of the need to provide them)
* The specified documents must be certified by the counterparty by affixing the signature of the authorized person (with the obligatory indication "The copy is correct"), full name, position, seal and date of certification of the following documents
