Comments: VimpelCom PJSC invites organizations to take part in the selection of suppliers for the provision of preliminary medical examinations for employees of the Southern Region for 2 years.
To apply for participation in the procurement event, you must provide the following completed documents (documentation package):
1. Documents for participation (Application form of the participant in PDF format (scanned with signature and seal) and Word; Compliance of the participant with the requirements of the procurement event in PDF format (scanned with signature and seal) and Word);
2. Financial analysis (Completed template in Excel and PDF format (signed and stamped scan); Scanned copies of the balance sheet for 2018, 2019, 2020, certified by the seal of the company and the tax authority; Scanned copies of the profit and loss statement for 2018, 2019 , 2020, certified by the seal of the company and the tax authority);
3. Counterparty Questionnaire (Completed file in PDF format (scanned with signature and seal) and Word);
4. Auction card (Completed file in Excel format);
5. Certificate of the Inspectorate of the Federal Tax Service on the absence of arrears on the submission of tax returns and payment of taxes (Document in PDF format (issued no earlier than 1 month before the date of submission));
6. The constituent documents of the organization (Certificate INN, PSRN, extract of the Unified State Register of Legal Entities / EGRIP; Charter; Order for the right to sign / power of attorney, if necessary).
Electronic copies of documents must meet the following conditions:
the participant must ensure that each scanned document is easy to read;
each scanned document must be saved in a separate file, while a document containing one page is saved in a separate file, and a document containing several pages and attachments must also be grouped into one file;
each document should have a name (abbreviated) corresponding to its content.
The package of documents must be sent to the e-mail address ZM4@rnd.beeline.ru (in the subject line, please indicate "Name of the participant / Competition of medical examinations").
The total size of the letter should not exceed 10 MB (if necessary, you can send several letters or several volumes of the same archive).

This invitation does not constitute a notice of an open tender or auction in the sense of Articles 447-449 of the Civil Code of the Russian Federation, and, accordingly, does not entail obligations for PJSC VimpelCom under these Articles.
