3. Work name:
3.1. Manufacturing works (from the materials of the Contractor) and installation
charge heater maintenance platform, in accordance with drawing No. ST13968 KM2.
3.2. Installation of the charge heater to the design site in accordance with drawing No. TX13968.
PS: The charge heater (Dec. No. HS08224) is provided by the Customer. Installation work, including the manufacture of the loading (Fig. No. 10322.00.00SB) and unloading device of the installation is carried out by the Contractor, from the materials of the Contractor.
3.3. Manufacture and installation of the charge heater drive group to the design position, in accordance with drawing No. HC08224.07SB.
PS: The metal structures of the drive group are made of materials
Contractor of works. The drive group (electric motor with gearbox) is provided by the Customer and is mounted by the Contractor.
3.4. Fabrication (from the material of the Contractor) and installation of the site at the elev. 7.00 in axes 15-16 and I-K, in accordance with drawing No. ST13968 KM2.
3.5. Manufacturing (from the materials of the Contractor) and installation of dust ventilation, in
according to drawing No. OV 020.13968.
PS: The works are made entirely from the materials and equipment of the Contractor.
3.6. Manufacturing (from the materials of the Contractor) and installation of air supply
charge heater, in accordance with drawing No. CB010.13968.
PS: The works are made entirely from the materials and equipment of the Contractor.
3.7. Manufacturing (from the materials of the Contractor) and installation of heat supply
heater VOT, in accordance with drawing No. TS010.13968. P.S.: The works are made entirely from the materials and equipment of the Contractor.
3.8. Pre-commissioning of all heater systems.
4. The purpose of the work:
Preparation of the mixing system for the installation of the Airich intensive mixer.

6. Requirement for the performer:
• The contractor must have the necessary production and human resources to perform this work.
• Production of works with possible exposure to harmful production factors: dust content, gas contamination, noise;
• Work in the congusted conditions of the existing workshop;
• Before issuing a work permit, the Contractor must provide the Customer with the following documents:
- a copy of the certificate (SRO) for the right to perform this type of work;
- copies of documents confirming the presence of an occupational safety service and/or an occupational safety specialist in the contractor (order for appointment to a position in a contractor, certificate labor protection specialist) or the existence of an agreement with an accredited organization in the field of labor protection services;
- a copy of the occupational safety briefing from the briefing log;
- POR or PPR or RTI;
- a copy of the workers' labor protection certification protocol;
- copies of certificates of certification of engineers on labor protection and general requirements of industrial safety;
- medical clearance of personnel.
The Contractor shall notify the Customer of the fulfillment of all these conditions in writing with all supporting documents attached. Failure to comply with the terms of this paragraph by the Contractor entitles the Customer to prevent the Contractor from performing the work, and the delay in performing the work in this case is the Contractor's fault. The participant who took part in the procedure (who made an offer) must upload the following documents to the system before the deadline for accepting applications:
1. ESTIMATE CALCULATION (without it the TKP is not considered)
2. Charter (with all additions and changes, if any).
3. Certificate of state registration.
4. Tax registration certificate.
5. Extract from the Unified State Register of Legal Entities
6. The balance sheet as of the last reporting date with the tax authority's note on acceptance.

— Decision (Protocol).

9. Certificate of Compliance with ISO 9001:2008 (if any) This request does not constitute a tender or auction notice and does not give any rights to participants, does not entail the occurrence of any obligations other than those expressly stated in the notice.
10. Requirements for the performer:
Before issuing a work permit, the Contractor must provide the Customer with the following documents:
- a copy of the certificate (SRO) for the right to perform this type of work;
- copies of documents confirming the presence of an occupational safety service and/or an occupational safety specialist in the contractor (order for appointment to a position in a contractor, certificate labor protection specialist) or the existence of an agreement with an accredited organization in the field of labor protection services;
- a copy of the occupational safety briefing from the briefing log;
- POR or PPR or RTI;
- a copy of the workers' labor protection certification protocol;
- copies of certificates of certification of engineers on labor protection and general requirements of industrial safety. When performing work on gas, on pressure vessels, using GPM, in electrical installations, hot work — copies of certificates for each type of work;
- medical clearance of personnel - copies of certificates of gas-electric welders, slingers, crane operators, hoists (towers). Copies of PTM certification protocols for gas-electric welders.
In case of any discrepancies in the information in the submitted documents with the requirements of regulatory legal acts, the Customer has the right to initiate an audit by the Health and Safety Service in matters of labor protection and safety Industrial safety in the Contractor's organization.
The Contractor shall notify the Customer of the fulfillment of all these conditions in writing with all supporting documents attached. Failure to comply with the terms of this clause by the Contractor entitles the Customer to prevent the Contractor from performing the work, the delay in performing the work in this case is the Contractor's fault.
The head of the contractor is obliged, prior to the commencement of work under the contract, to submit an appeal (in duplicate) containing a list of personnel by which work is expected to be performed under the contract. Copies of general civil passports and employment contracts (or civil law contracts) concluded with the persons specified in the appeal must be attached to this appeal. These copies of documents must be signed by the head of the contractor with a certified seal.
One copy of the submitted appeal together with the attached documents is sent to the Director of Security. The second copy of the application with the documents attached to it is sent to the head of the unit — the initiator of the conclusion of the contract.
In the absence of the contract organization specified in the first paragraph of this paragraph or the attached documents, as well as in the event of a discrepancy between the submitted list and the application the contractor for issuing passes, passes for employees of the specified organization are not issued. The delay in the performance of the work is attributed to the contractor. If there is a need to replace the employees specified in the appeal, the head of the contractor shall submit an updated list with the relevant documents attached to it.
Based on the results of work under the contract, the contractor, together with the KS-2 act, provides the customer (the head of the division — the initiator of the contract) copies of the SZV-M Form report" Information on Insured Persons" and a positive input control protocol on the transfer of this report to the territorial body of the Pension Fund of Russia, signed by EDS, for a period of time which is indicated in the report submitted by him in the KS-2 form.
In case of failure to provide SZV-M reports, or there is a discrepancy in the smaller direction of the number of contractor's employees specified in them, with the number specified in the appeal, KS-2 acts by the customer (head of the subdivisions - the initiator of the contract) are not signed, and the parties to the contract recognize the absence of the fact that the contractor has performed the work, the work under these acts is not subject to payment.
