LIST OF REQUIRED DOCUMENTS IN THE COMPOSITION OF THE APPLICATION FORM PARTICIPANT:
1. Application form for the participant (see Appendix 1 in the file Notice)
2. A commercial proposal that meets the requirements of the technical specifications, listing all the requirements of the technical specifications. When providing analogues - an indication of the positions to which the analogue is provided.
3. Statutory documents (charter, order on the appointment of the general director, order on the appointment of the chief accountant, certificate TIN, PSRN)
4. The lease agreement for the premises where the participant’s office is located.
5. The balance sheet for the last year, including the profit and loss statement, with the signature of the authorized person and the seal of the organization
6. Copy of the passport of the CEO (basic data and place of registration)
7. Documents confirming the Participant's compliance with the requirements of the Terms of Reference.

The documents specified in the Application must be signed by a person authorized in accordance with the legislation of the Russian Federation to act on behalf of the Participant without a power of attorney, or by an authorized person on the basis of a power of attorney. A copy of the Decision / Protocol (or otherwise) on the appointment of the Director General (or otherwise) as well as an Order on his / her assumption of office should be attached to the documentation.

Failure to provide the documents required in the Terms of Reference may cause a rejection of the Participant's application.

A commercial proposal must comply with the requirements of the technical specifications and contain:
1. A detailed specification and price (without VAT) for each item in the specification, as well as the total cost of goods / works / services.
2. Payment order.
3. Delivery time of the goods / work / service.
4. Warranty period for goods / work / services.
5. Willingness to start fulfilling an order under a letter of guarantee (if "Yes" is indicated in clause 4.1).
6. The participant’s questionnaire in the form provided in the form of a Word document and a scanned document with the signature and seal of an official whose personal data is indicated in the questionnaire.
7. Documents confirming compliance of the supplier with clause 3 of this Terms of Reference.
8. Other relevant terms and materials in accordance with the purchase request.
9. The validity of a commercial offer must be at least 2 months from the date of its submission.
