1. Request for participation in the request for proposals (Form № 1).
2. Vendor Questionnaire (Form № 2).
3. Table of technical and cost characteristics (Form № 3).
4. Certificate of performance of similar works (Work experience Form № 4) and letters of recommendation.
5. Commercial offer-in accordance with the requirements of the notice and technical
Job.
6. Statement of consent to receive/provide information by the Bank in the Bureau of Credit histories (for legal entities-form № 5).
7. Consent to the processing of personal data and receipt of credit history information (Form № 6).
8. Specification of the supplied equipment.
