The purchase documentation package includes the following forms to be filled out by the vendor:
1. Request for participation in the request for proposals (Form № 1).
2. Vendor Questionnaire (Form № 2).
3. Table of technical and cost characteristics (Form № 3).
4. Certificate of performance of similar works (Work experience Form № 4) and letters of recommendation.
5. Commercial offer-in accordance with the requirements of the notice and technical
Job.
6. Statement of consent to receive/provide information by the Bank in the Bureau of Credit histories (for legal entities-form № 5).
7. Consent to the processing of personal data and receipt of credit history information (Form № 6).
8. Specification of the supplied equipment.

This procedure of request for proposals is not an offer, an invitation to make an offer, acceptance of an offer or invitation to participate in tenders, its carrying out is not regulated by articles 447-449 of the Civil Code of the Russian Federation.
