2. All forms of documents are obligatory for filling. To participate in the procurement procedure it is necessary to fill in and provide:
2.1. A map of the offer and its annexes;
2.2. Annexes № № 1, 2, 3, 4 to the contract template for the execution of works:
- Annex 1 calculation of the contract price,
- Annex 2 Schedule of works;
- Annex 3 procedure for ensuring the work of materials, machines and mechanisms;
- Annex 4 conditions for the customer to assist the contractor;
2.3. Calculation of fuel demand for each unit of equipment and total quantity for the whole period of work execution;
2.4. Calculation of fuel component for each quote for performance of works according to ACC;
2.5. Calculation of additional costs for the mobilization/demobilization of machinery, machinery, equipment, staff delivery, nutrition and staff accommodation;
2.6. Extracts from the register of SRO members on the CMP with tolerances in accordance with the requirements of TK;
2.7. Documents for the proposed equipment (certificate of Ownership, lease/lease agreements, TCP, SAP);
2.8. Documentation for workers (industrial safety (attestation in RTN), labor protection, fire-technical minimum, work with GMP, Certificates Nax (for engineers minimum 2 level, for the head of Welding production 3 level, for welders 1 level );
2.9. Certificates and certificate of welding equipment in accordance with the requirements of the legislation of the Russian Federation (order RTN № 5 from 13.01.2015) at the facilities of GCO;
2.10. Valid certificates of verification of devices (measuring instruments);
2.11. Certificate of accreditation of the Laboratory for the documentary confirmation of the compacted ground;
2.12. Letter of recommendation (if available);
2.13. Package of constituent documents:
- Certificate of State Registration (Assignment of bin);
- Certificate of tax registration;
- A letter from the Federal Service state on the attribution of codes;
- Decision of the authorized body of the administration of the legal entity on the appointment of the Executive Body;
- Power of attorney for the person who will sign the contract (if necessary);
- Charter of legal entity (in the last version with all amendments and additions);
- Extract from the unified State Register of legal entities issued by the Russian Federal Tax Administration (for a period not exceeding 1 calendar month);
- Accounting (financial) statements for the last reporting year, or for the last reporting period, if the counterparty makes an interim accounting (financial) reporting, with the application of evidence of sending reports to the Tax inspectorate;
- Bank statement on the movement of funds for the last 3 months;
- Consent to the processing of personal data
- Copy of Passport, insurance Certificate of State pension insurance, filled-for individuals
3. Proposals will not be accepted without PODGRUZHENNOJ documentation (completed forms of documents, annexes under paragraph 2).
4. Criteria for choosing the best offer: Availability of necessary documents for performance of works, terms and cost of performance of services, terms of payment, availability of work experience.
5. This procedure of request for proposal is not a contest and its conduct is not regulated by articles 447-449 Ch. 1 of the Russian Civil Code;
6. This procedure is also not a public competition and is not regulated by articles 1057-1061 Ch. 2 of the Russian Civil Code;
7. Thus, this procedure of request for proposal does not impose on the customer an appropriate amount of civil liability.
8. The enclosed contract is not a public offer, but the terms of the contract will be binding on the winner of the bidding.
